Description
To deliver interventions as delegated by therapists, to support patients to
achieve their optimum level of independence and quality of life in the most
appropriate setting for the individual.


To assist patients to overcome their difficulties, build confidence, and improve or maintain functional ability and independence in everyday tasks under the supervision of a therapist.

To work on a rota system across 7 days.

To participate in multi-disciplinary teams promoting the role of rehabilitation and taking an active part in team projects.

To organise and manage a delegated caseload; obtain service data, analyse
where appropriate and bring forward ideas for service development.


An exciting opportunity has arisen for an enthusiastic and motivated individual to work as part of the Community Therapy Team to support a discharge to recover and assess pathway.

The team is committed to facilitating rapid hospital discharge and maximising independence within a patient's own home.

This post will involve some weekend working.
