Description

The post holder will provide a comprehensive service to the Trust with a focus on facilitating effective and timely discharge of patients no longer requiring acute in hospital care.


Working as part of the capacity and flow CBU, the post holder will work closely with wards to support and facilitate timely discharges, addressing barriers and delays to care and provide support to the discharge teams and site teams (site managers and capacity coordinators) in facilitating flow across the patient's inpatient journey.


The role requires a consistent and coordinated approach to the operational management of patient flow, with an emphasis on maintaining an on-going and 'real time' understanding of capacity and demand in managing site operations.

The post holder is required to support adoption of best practice flow methodologies including use of Red to Green principles, effective ward and board rounds and criteria to reside.


The post holder is required to work collaboratively with multidisciplinary teams across the organisation, as well as support the wider Capacity and Flow CBU in managing relationship across stakeholders on site operations.


  • To support the Capacity and Flow CBU in maintaining patient flow, working collaboratively with patient ward trackers across all inpatient areas to embed consistent and effective processes for the management of patient flow across the site.
  • To work with the ward trackers to routinely review, assess and escalate any barriers to delays that need to be addressed at an organisational level.
  • To work with the wards and support departments to ensure the use of Red to Green consistently to eliminate unnecessary delays for our patients.
  • To support the capacity coordinators in identifying and unblocking any delays to patient discharges on the day to support whole hospital flow.
  • To support the capacity coordinators and site managers in maintaining a real time capacity overview.
  • To work with the discharge team to support on planning for and facilitating complex discharge planning, working with ward teams to ensure all elements of a patients journey progress at pace.
  • Provide link in support with the transport team
  • As part of the capacity and flow CBU, work with and support the ward trackers to review patient flow data, identifying opportunities for improvement and reduction in length of stay through process and pathway changes.
  • Support the Capacity and Flow CBU in maintaining accurate records and documentation relating to patient flow, supporting completion of reports for use both internally and externally on site operations.
