    City West CMHT are looking for a qualified individual who has experience of supporting individuals with a range of complex mental healthneeds. Ideal candidates will also have experience of working with amulti-disciplinary team to support individuals and groups of people to recoverand gain skills to maintain their mental health and well-being.

    We want individuals working in our teams to demonstrate acommitment to a recovery approach and working collaboratively with the peoplewho use our services. We are also looking for people who can developrelationships and work in an integrated way with a range of practitioners fromoutside of the NHS including primary care, social care and thevoluntary-community sector.

    Main duties of the job

    The successful candidate will be responsible for organising and planning own caseload as an autonomous practitioner and monitoring those of other team members to meet patient priorities and service needs ( peer audits). To also work in a multi-disciplinary framework and to prioritise and organise workload effectively and use time management skills which may result in the requirement to readjust plans as situations change/arise such as crisis needs of patients/tribunal reports and Mental Health Act meetings.

    The post holder will respond to referrals from Primary care, outpatient and ward settings by undertaking comprehensive assessments of patients who may have complex needs (Mental Health, physical, emotional & psychosocial) using investigative, analytical and clinical reasoning skills to provide accurate reports of their condition and to feed this back to the professionals involved in their care both written and verbally.

    To receive clinical supervision on a monthly basis and supervise and co-ordinate less experienced staff and students working with the post holder, ensuring the standards of good practice are consistently achieved through leadership and supervision whilst providing advice, training and education to other members of the MDT/colleagues regarding nursing and evidence based interventions

  • Relevant profession qualification RMN, or equivalent
  • Experience in the delivery of psychosocial/psychologically informed treatments
  • Evidence of post basic training and development
  • Demonstrate the ability to: assess, plan, implement, evaluate and review packages of care as the Care Co-Ordinator/Lead Professional
  • Post registration experience working in mental health
  • Experience of working under pressure and managing own time
  • Excellent communication skills and an ability to work with other professionals, carers and organisations
