    Anexciting opportunity has arisen for a Nurse Practitioner/Advanced Nurse Practitionerat Fallodon Way Medical Centre.

    Wehave had ANPs successfully working within our GPteam for several years and are now looking to increase the balance of NursePractitioner support further. As well as considering experienced candidates forthis position, we are also able to support the training and development of asuitable candidate who has started the journey towards a Nurse Practitionerrole. In addition, due to current changes within our nursing team, we wouldalso welcome applications from ANPs wishing to develop management skills andwho are seeking opportunities to take a lead role across all aspects of thenursing team.

    Main duties of the job

    To provide a high standard of clinical supportto patients: diagnosing and prescribing treatments and drugs while workingwithin defined procedures and protocols as laid down by the GPs.

    We are a friendly, successful, training and research practice of over 11,500 patients, located in Henleaze, which is a leafy inner suburb of Bristol, with good schools and amenities. We have a relatively high elderly population but also a good mix with encourage and support ongoing professional development.

    The practice combines the best of traditional and innovative working, currently with 5 partners and 4associate practice is both proactive and reactive about managing the ever-changing workload in General Practice, offering a supportive environment and good work-life balance.

    The GP team is supported, in addition to the ANPs, by a Pharmacy team including two Practice Pharmacists, an Award-winning Care Coordination team, a Social Prescriber, First Contact Physio and mental health practitioner. There is a team of workflow administrators assisting the clinical team in managing inbound correspondence and results.

    JOB TITLE: ADVANCED NURSE PRACTITIONER

    REPORTS TO: THE NOMINATED PARTNER (Clinically)

    Roles/Specific tasks

  • Assess patients with undifferentiated and undiagnosed problems to diagnose and agree a treatment plan to meet their healthcare needs, providing a first point of contact for consultations with adults (aged 16 years plus) with an acute problem who wish to consult a clinician that same day. The consultation will include history taking, clinical assessment, diagnosis, treatment and evaluation of their care. It will demonstrate safe, clinical decision-making and expert care for patients within the general practice in an evidenced based, cost effective, individualised manner.
  • Instigate diagnostic tests of investigations as needed and interpret the consequent findings on receipt of results.
  • Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures and building effective team relations.
  • Triage of acute clinical telephone messages to resolve clinical questions where possible, and triage calls appropriately where not possible.
  • Make contemporaneous records of the consultation regardless of channel of consultation
  • Prioritise health problems, intervening as necessary, making appropriate referrals to secondary care and /or signposting patients to other services as needed
  • Promote health and well-being amongst the patient base of the practice through health promotion, education and screening programmes.
  • Participate in practice activities in relation to performance related work, including QOF, LES and DES activity.
  • Where the post-holder is an independent prescriber, prescribe safe effective medications to meet patient needs, as defined by current legislative framework and the local formulary.
  • Where the post-holder is not an independent prescriber, they should be prepared to gain this qualification (with the support of the practice).
  • Potentially participate in the pool of clinicians undertaking home visits.

  • Using personal security systems within the workplace according to Practice guidelines
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
  • Making effective use of training to update knowledge and skills
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
