Job title :
Care & Support Worker - Reablement

  • Provide appropriate physical, emotional, or intellectual support to individuals using services, enabling them to achieve their aspirations as determined through the individual care and support planning process. This includes both planned and unplanned care as well as responding to attendant call systems.
  • To ensure that each individual's dignity and privacy are respected, independence is maximised at all times.
  • Provide personal care to individuals according to their individual care plans, which may include assistance, support, encouragement, and tasks such as washing, dressing, grooming, continence care, and associated manual handling procedures.
  • To attend to the healthcare needs of individuals, as outlined in their individual care plans and within a competency framework. This may include administering medications, using equipment to monitor physiological metrics, and performing simple wound dressings as part of a treatment plan.
  • To support individuals in accessing a range of community opportunities, including those related to leisure, education, and employment, as directed by line managers.
  • To actively participate in the assessment and care planning process for individuals by engaging in discussions, review processes, and attending meetings.
  • Maintain and update records related to the ongoing assessment and monitoring of individuals using services in accordance with organizational policies and procedures.
  • To communicate and collaborate with a range of stakeholders to promote effective service delivery.
  • To perform any other duties and responsibilities as assigned, commensurate with the job grade.

Job Type:
Part-time


Salary:
£13.84-£15.51 per hour

Expected hours: 30.00 per week

  • Monday to Friday

  • Day shift

Ability to commute/relocate:

  • Widnes, Cheshire: reliably commute or plan to relocate before starting work (required)

Experience:


  • Home care: 1 year (preferred)

Language:


  • English (preferred)

Licence/Certification:

  • Driving Licence (preferred)
