    Job summary

    Lead on all aspects of medicines optimisation and safety across the Provide Group including:

  • Ensuring all medicines optimisation and pharmaceutical processes undertaken across Provide services are safe, effective and evidence based.
  • Act as the Medication Safety Officer (MSO) and Lead on medication safety, supporting risk aversion and management
  • Lead on Non-Medical Prescribing ensuring safe prescribing practices across Provide Group services
  • Maintain responsibility for Medicines governance and assurance across the Provide Group
  • Line Manage the Band 7 Governance Lead Medicines post and maintain oversight and assurance of the contracted pharmacy service for all Virtual Wards
  • Main duties of the job

    KEY RESPONSIBILITIES

    Overall purpose of job:

  • Maintain safe and effective medicines management processes across the Provide Group
  • Assess and quantify risks associated with medicines and action to minimise risks as much as possible
  • Identify significant risks associated with Pharmacy and Medicines and ensure appropriate escalation and risk management including controlled drugs and unlicensed medicines
  • Maintain the NMP Database, ensuring NMP Governance and safe prescribing
  • As the Medication Safety Officer, Investigate and manage medicines incidents ensuring appropriate escalation and communication across Provide Group and the MSE via the Discharge Outcomes Group
  • Active involvement in the development of Collaborative and System wide patient pathways and redesign to ensure effective processes are adapted in relation to medicines optimisation
  • To deputise for the Assistant Director Pharmacy and Medicines as required
  • To support and participate in clinical research and audit, overseeing the Pharmacy and Medicines annual clinical audit programme

    Job responsibilities

    KEY AREAS OF RESPONSIBILITY

    Communication and Relationships

  • Ensure timely provision and presentation of written and verbal reports in relation to medicines governance, assurance and risk management to all levels across Provide Group
  • Develop, maintain and support communication across the MSE footprint, working closely with Collaborative partners and wider MSE colleagues to agree actions and system wide requirements for Pharmacy and Medicines and supporting the AD Pharmacy and Medicines to lead on Community Collaborative medicines optimisation
  • Motivate, persuade and negotiate with key internal and external stakeholders to implement new initiatives (internal and system wide) and NHS England directives to promote new ways of working, ensuring a collaborative response
  • Provide expert clinical, therapeutic, legislative and best practice advice to all Provide Group services and the Community Collaborative, promoting safe and effective medicines use, including clinical advice in areas where information is lacking and where medical or other opinions differs
  • Work collaboratively with all the local partners which include health and social care agencies, third sector and the independent organisations to ensure a unified and coordinated approach around the development of seamless pharmaceutical care for patients

    MPharm qualification or equivalent
