    Description

    We have an exciting opportunity to join our dynamic team and lead the charge in ensuring seamless patient transitions from the acute to the community.

    As a Senior Discharge team lead, you will steer coordinated discharge efforts for simple and complex cases, actively prevent delays in discharge and patient flow and ensure patients receive the care they need as appropriate.

    This is an exciting opportunity to collaborate with external agencies, building strong relationships to guarantee continuity of care.

    Main duties of the job
    • Lead the co-ordinated support of discharges within specified wards, including complex discharges that may otherwise result in Delayed Transfers of Care (DTOC)/Medically Ready for Discharge (MRD). Working specifically with the discharge coordinator in partnership and facilitating simple discharges.
    • Liaise with external agencies and organisations
    • Develop effective links and relationships with relevant professional partners
    • Organise meetings involving healthcare professionals, patients, and their families
    • Provide comprehensive information on all discharge arrangements and services to patient and family/carers, both verbally and in writing.
    • Assess, monitor and report on patient length of stay, delayed transfers of care and practice related issues.
    • Assess, monitor and report on criteria to reside and delays reporting, initiated by the MDT at the board rounds. To evidence the trust is adhering to report SITREP data.
    • Proactively support and lead the drive for discharges during critical times such as episodes of Business Continuity.
    • To line manage, develop and support the Discharge Co-ordinator team.
    • Assist in the collection and the auditing of discharge data and be prepared to account for: - Additional bed-days beyond EDD, noting carefully the reasons for any delayed discharge. - Repeated extensions of EDD - Cases of 'failed discharge' or readmission, within 48hrs of patients with the same condition.

    Person Specification Essential and Desirable Essential
    • NMC registered Nurse or Registered Allied Healthcare Professional
    • Substantial experience at band 6 or equivalent management responsibility in health or social care.
    • Detailed understanding of discharge planning processes within an acute setting, including community care, continuing health care and mental capacity issues
    Desirable
    • Recognised teaching/ mentorship course
    • Proven track Record in managing teams
    • Excellent communication skills
    • oProven ability to analyse complex problems and to develop and successfully implement practical and workable solutions to address them.
