- Lead the co-ordinated support of discharges within specified wards, including complex discharges that may otherwise result in Delayed Transfers of Care (DTOC)/Medically Ready for Discharge (MRD). - NMC registered Nurse or Registered Allied Healthcare Professional
- Substantial experience at band 6 or equivalent management responsibility in health or social care.
- Detailed understanding of discharge planning processes within an acute setting, including community care, continuing health care and mental capacity issues
- Recognised teaching/ mentorship course
- Proven track Record in managing teams
- Excellent communication skills
- oProven ability to analyse complex problems and to develop and successfully implement practical and workable solutions to address them.
- Evidence of having undertaken own development to improve understanding of equalities issues
- Evidence of having championed diversity in previous roles (as appropriate to role).
