- Provide a comprehensive clinical pharmacy service to the renal ward
- Participate in renal transplant clinics and address patient medication queries
- Act as the point of contact for various nephrology areas
- Undertake APKD clinic tasks and manage related prescriptions
- Write and review SOPs, PGDs, and drug-related guidelines
- Liaise with NHSE and ICS to ensure drug availability
- Responsible for clinical screening of homecare and renal BDU prescriptions
- Respond to medication queries from the team and outpatient pharmacy
- Current registered practising Member of GPhC
- Postgraduate Diploma in Clinical Pharmacy
- Independent Pharmacist Prescriber or willingness to obtain
- Masters Degree in Pharmacy or equivalent
- Advance Clinical Practitioner
- Accredited Diploma Tutor
- Extensive Hospital Pharmacy experience, including substantial time in a UK acute hospital setting
- Clinical pharmacy experience at the ward level
- Collaboration within multidisciplinary teams
- Experience in developing protocols and PGDs
