- To provide holistic and individualised care to patients and carers within the Primary Care Cardiac Service

- To plan, deliver and evaluate individual cardiac rehabilitation packages, managing heart failure patients liaising and referring on appropriately to all other relevant agencies and services.

- Supporting group sessions of cardiac rehabilitation as appropriate within the community.
- Plan, deliver and evaluate programmes of cardiac rehabilitation to address peoples health and well-being needs. Support patients with heart failure supported by specialist heart failure nurses.
- Provide secondary prevention strategies to TIA patients to reduce cardiovascular risk
- Contribute to the public health agenda by raising awareness of health and well-beingneeds to individuals specifically raising awareness of cardiovascular disease
- Contribute to the development of new services to meet the needs of patients withCardiovascular Disease.
- Implement the provision of evidence-based/ best practice.
- Actively encourage patient and carer feedback about the Primary Care Cardiac Service
- Maintain a high standard of care to patients and carers.
- Contribute to the continual improvement of the service.
- Maintain a professional image as outlined in NMC code of conduct.
- Contribute to the Clinical Governance agenda and apply risk management systems Participate in relevant research and audit activity.
- Promote and maintain good practice in record keeping by self and the team to ensure CityCare and Professional standards are met.
- Develop and maintain close working relationships with other statutory bodies, community groups and voluntary agencies with the aim of reducing the incidence of cardiovascular disease and improving access to services in high-risk groups within local communities.
- Plan and take part in health promotion projects.
- Be responsible for own time and caseload management
- Support effective team working across the Integrated primary care cardiac service and promoting MDT working.
- To manage complex situations with a vulnerable client group. To respond rapidly to the constantly changing environment and client needs. To continually risk assess situations.
- Educate, train and advise other health and social care workers, students and any other person involved in working with cardiovascular disease clients.
- Be responsible for the development of own knowledge and clinical practice andrecognises ones own limitations.
- Act as a mentor to a variety of nursing students
- Participate in the development of other team members.
- Participate in the supervision framework as a supervisee and supervisor.
- Maintain a portfolio to demonstrate continuing professional development
- Contribute to the implementation of service developments.

- Attend mandatory training and keep an up to date record of attendance.
- Promote and maintain best practice that ensures the health safety and security of self, patients and colleagues in accordance with the policies procedures and guidelines of Nottingham CityCare Partnership.
- Work as an autonomous practitioner, in line with Professional Standards, seeking advice from senior staff as appropriate and having an awareness of own development needs.

- RN qualification on part 1 of the register
