    Job summary

    This is an exciting opportunity to join the Merseycare Long COVID Tier 3 Service.

    The post holder will provide an assessment of patients in this service to support the caseload of clients with Long COVID symptoms using evidence based and client-centred principles to assess, plan, implement and evaluate interventions.

    The successful candidate will be expected to carry out all relevant forms of care, providing highly specialised advice and physiotherapeutic input for the treatment of this disease.

    You will play a vital role in supporting the delivery of integrated, co-ordinated, comprehensive, seamless, person-centred care. As a member of the Tier 3 Long COVID Assessment & Treatment service Multidisciplinary Team (MDT) you will be required to make recommendations based on the principles of shared decision making for people living with or experiencing the impact of Long Covid symptoms and access Tier 3 Services.

    This will be delivered across various locations in the community for Merseycare Long COVID Services.

    Main duties of the job

    As a member of the Tier 3 Long COVID Assessment & Treatment service Multidisciplinary Team (MDT) you will be required to make recommendations and will always take account of the views, preferences and circumstances of patients/service users, parents or carers when considering clinical and non-clinical decision-making.

    The post holder will work as a key member of the MDT in the assessment and management of a caseload of clients, and plan, deliver and evaluate the treatment needs of clients with a variety of complex social and health care needs.

    The post holder will participate as an active member of the multidisciplinary team in developing and delivering high quality and innovative services to the local population, to promote effective teamwork within Mersey Care delegating activities appropriately, to the benefit of the patient and service delivery. The role is not limited to, but will include, the triaging of inbound referrals, providing pulmonary assessments, breathlessness management, bespoke exercise programmes, facilitating of patient groups, supervision of staff, and day to running of the service in the team coordinator's absence.

    The post holder must be flexible as the role will include working from a Liverpool base and include various sites, will at times require home visits, video assessments, and working remotely from home. Providing support to other teams may also be required when and where appropriate.

    Job responsibilities

    The expectations within this job description will be achieved through hands-on clinical practice, education and training and research and duties will include:

  • To undertake timely, holistic assessments of patients, involving functional and environmental factors.
  • To be able to undertake assessment and interventions within the Long COVID Tier 3 service.
  • To be able undertake assessments autonomously as well as working with other members of the multidisciplinary team (MDT).
  • To provide comprehensive intervention to patients making use of assessment and treatment facilities where appropriate.
  • To be able to make rapid decisions where required, using clinical reasoning skills, knowledge and experience, with support as appropriate.
  • To be responsible for a clinical caseload working within multidisciplinary teams, sharing care appropriately ensuring patients and carers receive optimum level of therapy and holistic care within available resources.
  • To provide supervision to staff within the team.

    Qualifications

    Essential

  • Relevant qualification in Physiotherapy Current registration with HCPC as a Physiotherapist
  • Experience and/or additional education in relevant field to post gradual diploma or equivalent level, combination of education, further training, and experience
  • Evidence of CPD relevant to treatment of older adults, physical disability, and rehabilitation
  • Evidence of an up to date CPD portfolio
  • Desirable

  • Qualification in team / service management or prepared to work towards
  • Masters level study related to rehabilitation or prepared to work toward
  • Membership of the CSP
  • KNOWLEDGE/ EXPERIENCE

  • Evidence of recent post-graduate experience in core areas including elderly care, respiratory and rapid response
  • Experience of working in a community/ primary care setting
  • Experience of negotiating and liaising within a multidisciplinary/ interdisciplinary environment
  • Experience of working and assessing people with complex needs and delivering complex care plans
  • Experience of coordinating and managing a team
  • Experience of supervision and teaching others
  • Active participation in in service training
  • Experience in service development
  • Knowledge of principles of care for long term conditions, relevant National Service Frameworks, NICE and other national standards including the out of hospital strategies
  • Knowledge and understanding of current health care policy and relevance to the delivery of healthcare services
  • Knowledge of Primary Health Care and collaborative working with other care providers, , social care, acute hospitals, and voluntary services
