    Senior Nurse Practitioner – Community Stroke Rehabilitation Team

    An exciting opportunity has arisen for a specialist nurse to join our award-winning community multidisciplinary stroke team. This is a 6 month fixed term Band 6 post, & is an opportunity for internal secondment.

    We are looking for a nurse with experience & keen interest in neurological rehabilitation to join our team in providing high quality, client-centred, holistic rehabilitation for patients who have had a stroke living within Lambeth & Southwark.

    You will be a nurse with postgraduate experience, ideally with some community experience, working with adults with acquired neurological impairments. You will work hrs per week Monday to Friday.

    We can offer you a supportive, friendly working environment where patients come first & team members feel listened to, valued & appreciated. You will be well-supported through supervision & appraisal; & will have access to in-service training within the community neuro team as well as the wider GSTT nursing community. You will also receive clinical supervision from an Advanced Nurse Practitioner, based on the Stroke Unit, St Thomas' Hospital.

    You do not need to be able to drive for this post. If you do drive, pool cars available, or you can use your own car. You would otherwise use public transport, own bicycle, or walk. Travel expenses are reimbursed.

    Main duties of the job

    The Community Stroke Team provides goal orientated multi-disciplinary rehabilitation within the community environment following hospital discharge. The Stroke multi-disciplinary team (MDT) includes Clinical Psychology, Physiotherapy, Stroke Care Advisor, Stroke Reviewer, Occupational Therapy, Speech and Language Therapy, multi-disciplinary Rehabilitation Support Workers / Assistants, and Nursing.

    The post holder will:

  • Provide a specialist nursing service to clients as part of the multidisciplinary Stroke Rehabilitation service in the communities of Lambeth and Southwark.
  • Work closely alongside other members of the MDT to ensure a coordinated approach to client care.
  • Participate in monthly supervision and annual appraisal.
  • Be a source of specialist nursing clinical expertise and advice on client care.
  • Building partnership with GPs, District Nurses and Specialists Nurses in the community.
  • Support the development of RSW.

    Detailed job description and main responsibilities

    Professional

  • Work as an autonomous practitioner complying with the standards set by the regulatory body (Nursing and Midwifery Council), professional body (Royal College of Nursing), government, Trust and department; thereby being professionally and legally responsible and accountable for all aspects of own work and to meet the requirements for professional registration.
  • Work as an autonomous lone practitioner including case-managing clients.
  • To be involved in the day to day planning and organising of the care delivered to clients on team caseload, ensuring the co-ordination of care with relevant team members so care is provided promptly and to a high standard
  • To carry nursing assessments to prevent hospital admission or re-admission and facilitate early discharge home from hospital.
  • Be able to identify significant health changes that would warrant medical intervention and liaise with the relevant practitioner.
  • Maintain close liaison with other senior colleagues in the team and ensure effective communication with all team members for day to day smooth running of the service.
  • Keep abreast of current issues within the profession, taking responsibility for personal growth and development reading relevant journals and research papers and participating in appropriate work journal clubs and related research / evaluation projects.
  • Maintain and record ongoing continuing professional development, using a professional portfolio.
  • Attend professional body / clinical interest group meetings as appropriate.

  • Attend training courses, participate in experiential learning as identified in PDP and impart information to colleagues.

  • Provide a comprehensive assessment, treatment and management service to a caseload of adult patients who have experienced stroke, following local operational policies and agreed standards and guidelines.
  • To be involved in the day to day planning and organising of the care delivered to clients on team caseload, ensuring the co-ordination of care with relevant team members so care is provided promptly and to a high standard.
  • To carry nursing assessments to prevent hospital admission or re-admission and facilitate early discharge home from hospital.
  • Be able to identify significant health changes that would warrant medical intervention and liaise with the relevant practitioner.
  • Provide specialist interventions based on comprehensive risk assessment, evidence based practise and sound clinical reasoning.
  • Monitor effectiveness of interventions through goal setting, outcome measurement and evaluation of practice.
  • Be responsible for prioritising and managing own workload. To achieve an agreed expected individual activity target (face to face contacts).
  • Be responsible for delegating appropriate tasks to RSWs and be accountable for own professional practice and that of the support staff / students delegated to.
  • Work collaboratively with clients and carers, members of the MDT and external agencies to ensure a co-ordinated approach to service delivery.
  • To discuss, advise and plan appropriate goals with clients, carers, and the MDT.
  • Empower individuals to take responsibility for self-management during the treatment period and following discharge.
  • Educate individuals, carers and family members of the impact of stroke on physical, cognitive and psychological wellbeing and actively promoting healthy lifestyle, contributing to stroke prevention.
  • Respect clients' dignity, privacy, culture and wishes during treatment, with a commitment to equal access to rehabilitation.
  • Attend all appropriate team, professional and other meetings related to clinical caseload.
  • Participate in the Trust staff appraisal scheme.
  • Be responsible for safekeeping and cleanliness and maintenance of equipment ensuring standards of infection control are met and reporting all equipment defects, accidents and complaints to the Clinical Pathway Manager.
