Scale 10-15 £ £25309 per annum

Holidays: 28 days + Bank holidays pro rata.

Hours of work: 37 Hours per Week.

All staff are responsible to the Lincolnshire Community and Voluntary Service via the Chief Executive.

LCVS are looking for a
Social Prescribing Link Worker to join our team. Link Workers are part of a multi-disciplinary team within Primary Care and the wider Neighbourhood Team.

We are looking for an organised, self-motivated, and confident individual to join our established Social Prescribing team working with and helping adults access the support they need to live independent and healthier lives.

Applicants must have experience of working with adults and have a background in health and/or social care or experience of supporting people's wellbeing in a community setting.

You must be a good listener, have time for people and be committed to supporting local communities to care for each other.

You should have experience of working positively with people facing social and emotional challenges as well as having great interpersonal skills and the ability to support people, community groups and local organisations.

You will work alongside multiple GP practices within a locality, independently managing a caseload and new referrals, providing personalised support, and connecting participants with health care providers, community support and services to improve independent living, health and wellbeing.

It is essential that you understand how to prioritise need, be able to recognise when to refer on to specialist services, being confident in your ability to communicate at different levels with people from all backgrounds and across both the health and social care sectors.

You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work.
