    Job summary

    We have an exciting and rare 12 month fixed term contract opportunity for a Therapy Assistant to join the South East Staffordshire Community Rehabilitation team.

  • A Therapy/ Rehabilitation Assistant with experience of caring for patients in the community with long term conditions.

  • Wanting to play your part in improving services in South East Staffordshire for people with a diagnosis of Stroke
  • You will be part of a small team who have been selected to support a Stroke project (funded by Squire - Stroke Quality Improvement for Rehabilitation) over the next 12 months. This post will support the scoping of a South East Staffordshire Stroke pathway for 6 months followed by delivery of a pilot pathway and the first 6 months of the project we will support you to develop your skills in the treatment of Stroke patients in a clinic setting. During this time, you will get used to the neuro services, systems and processes in readiness for the next phase. The second phase will be to support the MDT in providing treatment to Stroke patients. By the end of the 12 months you will have gained valuable experience which can be transferred to many other therapy roles. You will have played an important role in the development of Stroke services within South East Staffordshire.

    A car driver is essential for this post due to the nature of our work.

    Main duties of the job

    You will be

  • Working with the Stroke Coordinator to identify and co-ordinate the Stroke Care Pathway for all adult stroke patients, networking and working with relevant services.
  • Working with the team on the co-ordination of all Stroke referrals made to the Community Rehabilitation Team.
  • Contributing to development of the stroke rehabilitation service in line including research, innovation, service design and improvement providing assurance of efficiency and effectiveness in line with national standards.
  • Liaising effectively with key front line practitioners and across the Acute Trust, Rehabilitation and Community Teams, District Nursing, GPs, Social Services and the independent sector.
  • Supporting the team developing stroke aftercare and review in a community setting for stroke survivors, family members and/or carer. Supporting and working alongside voluntary organisations Stroke Association / Headway etc.
  • Supporting stroke patients in the community following their initial episode of care.
  • Ensuring the views of patients and carers are taken into care provision through effective involvement at an early stage and adopting a care co-ordination approach

    Clinical

  • To establish and maintain effective working relationships both within the Partnership Trust and with external organisations in order to meet service requirements and support and demonstrate the Trusts equality values.
  • To contribute to the assessment and implementation of programmes of treatment care/support for individuals, families and carers under the guidance of a registered The care/support plans may include the following: Promoting independence/self-management and maintaining safety Increased functional ability Improvement in health and wellbeing including mental health Interventions in disease/condition management Prevention and reducing of health inequalities Admission avoidance Early supported discharge Contribute to case management/ treatment care plans Palliative and end of life care

  • To maintain and develop skills and knowledge to deliver a range of interventions under the guidance of a registered practitioner and set an example of good practice to other team members including skill sharing. To provide therapy support to service users within specified areas.
  • To maintain sensitivity at all times to the emotional needs of the service user and their carers offering reassurance and empathy. This may be in regard to the nature/prognosis of their condition and implications of the same.
  • To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be
  • To apply skills, knowledge and competences to recognise any significant change in the holistic needs of the service user/carer and feedback to the registered practitioner.
  • To maintain contemporaneous and accurate records as required by the Partnership Trust and in line with HCPC and CSP standards.
  • To take responsibility for the safe use and transportation of equipment between locations. To work in non-purpose built rooms involving the necessity to move furniture, therapy equipment etc. This may be required on a daily basis or at least once a week.

  • To use problem solving and innovation to deliver a high standard of intervention within competences whilst managing the demands of unplanned aspects of the role where appropriate.

    Essential

  • NVQ level 2 in health related subject and/or equivalent experience
  • Evidence of/willing to undertake training relevant to the post.

  • Relevant placements/experience to the role.
