    Description

    An exciting opportunity has arisen for a passionate and enthusiastic Mental Health Practitioner to work within the Bury Community Mental Health Team.

    The post holder will be expected to demonstrate high level of commitment, motivation and knowledge of working with people with severe and enduring mental health problems.

    The role will require clinically effective assessment, care management skills and interventions in accordance with the Care Programme Approach and Care Act framework.


    The post holders duties will include engagement with clients in the development of person centred, recovery focused care plans, supporting our experienced duty team with short-term pieces of work and supporting people managing crisis.

    You will be dedicated to delivery, both as part of a team and working independently, of a high-quality, innovative, recovery focused and person centred care which meets the needs of people who use our service and their carers.

    The successful applicant will work effectively with adult service users with mental health needs across Bury.

    The post holder will be highly skilled and competent in both holistic health assessment and identifying social needs, risk management and Formulation, developing collaborative care plans, facilitating the implementation of the agreed plan for care with a focus on recovery and social inclusion.

    The Practitioner will provide comprehensive mental health service to adults of working age who experience moderate to severe mental health problems in a community setting.


    • Utilise the Care Programme Approach to support and promote the recovery of service users under the care of the service.
    • Working collaboratively across a range of disciplines to continuously promote a needs based, recovery focused approach to all service users.
    • Offer support as needed as part of Duty Rota.
    • The Team offer a Monday-Friday service within the hours of 9am 5pm.
    • Flexible working can be discussed in line with Trust Policy.
    Short courses and experience to post-graduate diploma level

    Psycho-social / CBT qualification / other therapy qualification

    Experience of working within Community Mental Health Setting

    Experience of working within Community Mental Health Setting as a Care Co-ordinator
    Experience of Safeguarding

    Mental Health and Social Care
    Care Programme approach
    Mental Health legislation
    Knowledge of severe and enduring mental health problems, including personality disorder

    Recovery Model ? Care Act

    Ability to undertake the Care Coordinator role
    Ability to supervise and support Band 5's Community Support Workers in meeting the needs of clients on their caseload
    Ability to provide professional expertise in the care of mental health clients
    Ability to provide appropriate education and training to peers and assistants as appropriate.
    Ability to value diversity and respond constructively to discriminatory behaviour
