- Awareness of harassment, social exclusion issues and the effects of person and institutional discrimination
- Knowledge/awareness of relevant legislation and guidance including common law issues, the Mental Health Act (1983), Community Care Act (1990), Human Rights Act (1998), Criminal Justice Act (2003)

Job summary
This is an exciting opportunity to work as part of the newly expanded Health and Justice Service in Cheshire. The post will be working within the prison establishments across the footprint of Cheshire including HMP Styal, HMP Risley and HMP / YOI Thorn Cross.
The community support offered will aim to support service users to engage with services such as physical health, mental health, housing etc. and will include support to attend appointments with agencies referred to supporting an effective transition into the community and health equalities.
The successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a difference.
This role is subject to Prison Vetting.

The Reconnect Service seeks to improve the continuity of care of vulnerable people leaving prison by working with them before they leave and support their transition to community-based services, there by safeguarding health gains made whilst in prison.
The Reconnect service aims to improve the wellbeing of people leaving prison, reduce inequalities and address health-related drivers of offending behaviours.
Whilst not a service which provides clinical interventions, the Reconnect Service offers liaison, advocacy, signposting and support to those leaving prison to support engagement in community-based health and support services.

Job descriptionJob responsibilities

Contribute to and assist in the delivery of a needs led Reconnect Service and contribute to the daily functioning of the team.
To undertake initial assessments of service users needs following referral to the service and to develop a plan of support in conjunction with the service user and the wider Reconnect Team.
Be actively involved in the screening and assertive follow up of individuals with a range of vulnerabilities. Working with Reconnect practitioners and criminal agency staff, social care, health and voluntary, charitable agencies.
To carry a caseload of service users with a range of complex vulnerabilities.
To ensure support plans are formulated acted upon and reviewed.
To effectively liaise and develop links with services within the prison setting and, in the community, to establish effective through care and after care pathways.
To effectively plan discharges and prison releases in conjunction with community services.
Successfully time manage and prioritise areas of responsibility.
Attend relevant team/service and wider meetings as required and appropriate, helping to promote new ideas, methods and assist in their implementation.
Communicate effectively within own service and with other colleagues.
To maintain accurate, appropriate and contemporaneous records in accordance with the information governance, data requirements and Trust Policy.
