The Therapy and Wellbeing Centrehas recently opened here at Katharine House Hospice. We now run an Outpatientservice where patients can be seen by a variety of multi-disciplinary teammembers. Our aim is to improve quality of life by providing support andspecialist care.
We are seeking a motivated,enthusiastic individual with excellent standards of patient care andoutstanding communication skills to join us and be part of our team.

You will be aqualified to level 3 in NVQ Care and must have at least 1 years relevantexperience in a healthcare setting.

Main duties of the job
The post holderwill work as a member of the multi-disciplinary team assisting with patient careactivities under the supervision of a registered nurse. They will support thedelivery of direct patient care by providing clerical, catering and housekeepingassistance as required, without the direct supervision of a registered nurse.

Summary
To support allpatients and carers attending the TWC.
Specific Objectives
1. To participate fully in the deliveryof care to patients as delegated by aregistered nurse or allied health professional
2. To support patient/carers via telephonesupport calls.
3. Support staff to deliver care/ social sessions within the TWC.
4. To undertake housekeeping duties asrequired
5. To provide clerical assistance asrequired
KeyTasks and ResponsibilitiesTo meet the above broad objectives, the post holderwill have responsibilities in four main areas of work as follows:

Clinical
1) To participate fully in the delivery ofcare to patients as delegated by registered nurses and allied healthprofessionals physiotherapist, occupational therapist, ensuring thatpatients are helped to maintain their dignity, privacy and individuality.
2) Manage a telephone support caseload
3) Identify changes in patient conditionsand, within their capabilities, recognise where significant changes in the planof care are required and report these back to the nurse on duty in a prompt andtimely manner.
4) Liaise effectively with other nursingand multi professional team members within the scope of their role.
5) To communicate relevant informationregarding patients and carers to registered nurses, verbally and in writtenform.
6) Completing clinical observations asrequested
7) Ensure the correct usage and maintenanceof all equipment
8) Maintain patient written records inaccordance with the wishes of registered nurses
9) To provide a welcome for patients andvisitors to the centre, providing assistance, support and information asrequired
10) Demonstrate initiative in planning dailyand weekly routines
11) Maintains up-to-date knowledge ofchanges in policies and procedures relevant to the post.
12) To ensure all clinical areas are cleanand tidy and in line with current Infection Control Guidance.
Educational
1)To ensure all staffcomplete Mandatory training as set by KHH.
2)Encourage Staff intheir own professional development.
3)Supportingstaff in the delivery ofawareness and condition education sessions to patients and carers
Clerical
1) Answer the telephone and take messagesin accordance with hospice policy
2) Maintain stock levels of clinical formsand patients record forms as needed
3) Participate in filing patientinformation as directed by registered nurses
Personnel
1) Take all measures to ensure the safetyand welfare of all patients and staff, having due regard to the Health andSafety at Work Act
2) Attend statutory updates as required bythe Care Quality Commission
3) Actively seek support when needed andcontribute to providing support for others when needed
Personal Development
Katharine Housewill support the continuous development of the post holder who is expected tobe proactive in ensuring their own clinical supervision and identifying theirown developmental needs.

Infection Prevention and Control
In accordance with the Health and Social Care Act2012, the post holder will actively participate in the prevention and controlof infection within the capacity of their role.The Act requires the post holder to attend infection prevention andcontrol training on induction and at regular updates and to take responsibilityfor the practical application of the training in the course of their work. Infection prevention and control must beincluded in any personal development plan or appraisal.
