Job summary
We are looking for enthusiastic, innovative mental health practitioners either from a background in Mental Health Nursing, Social Work or Occupational Therapy who are committed to the principles of recovery.
The role of the care coordinator is to have an agreed caseload, working collaboratively with service users and carers. These include psychological interventions, medication support, prescribing, access to group work and carers support. To engage service users creatively, develop collaborative care plans including risk management plans.
You will need robust assessment skills to enable the team to deliver high quality care and treatment plans. Excellent communication skills are also essential to enable you to be a role model for others, as well as positively representing the team and the organisation as a whole.
Main duties of the job
You must also be able to liaise effectively with other services, both internal and external to Southern Health Foundation NHS Trust, retaining service user focus at the heart of any clinical decision making to ensure the best outcome.
If you are a highly motivated, enthusiastic, flexible, resourceful and resilient individual who is committed to providing care with compassion to service users in their recovery then Mental Health Community Team is the place to work.
We also actively encourage band 5 applicants who may have less experience, to consider our bespoke development posts. We will individualise a program of support and development to work through with the aim of progressing to a band 6 within an agreed time period.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
