The Recovery and Rehabilitation Team is integrated with Croydon Social Care in working with all adults in Croydon Funded Placements experiencing severe and enduring mental illness and who present with complex needs. This advert is for 2 vacancies in Recovery and Rehabilitation Team and are for contracts with SLAM .
Main duties of the job
The post holder will work as Care Coordinator for a caseload of service users in funded Placements such as Residential homes, Nursing homes, supported living, etc. who have complex mental health difficulties, and work with their carers where applicable. The post holder will be expected to have excellent self-management skills, and work collaboratively within the team. The post holder must be able to formulate flexible, robust and holistic care and recovery plans with service users and/or carers, to work well under pressure, be experienced in dynamic risk assessment, and to have excellent communication skills.
We also run our own Clozapine clinic and nurses will be encouraged to use or gain phlebotomy skills and to train on PocHi as well as support the administration of medications including Depot injections.

1. Working autonomously within the parameters of the role leading and supporting junior staff and wider care team to implement the delivery of quality nursing, Social Work, or OT care, contributing to the physical, psychological, social, recreational and spiritual care in a recovery-focused way through therapeutic engagement and activities, and taking steps to address any issues that arise.
2. Leading on and undertaking activities to ensure that the fundamentals of care are met - nutrition, hydration, hygiene, comfort, emotional and social support:

18. Protecting service users, self and others by maintaining a clean and healthy environment in accordance with health and safety and infection control policies. Ensuring safe working practices and working environment including monitoring and maintaining cleanliness of the service area and report, without delay, all hazards - real and potential.
