    Job Title: Advanced Nurse Practitioner at Pontefract & Knottingley PCN

    This unique opportunity is a new and exciting position within the PCN. We are seeking an experienced ANP to join our Care Home Team, which consists of both clinical and non-clinical staff covering nine care homes in the district. Your role will involve driving the development and enhancement of processes to ensure the delivery of high-quality planned care for care home patients and to minimize unnecessary hospital admissions.

    Main Responsibilities:

    • Collaborate with the Care Home Team to deliver patient-focused care.
    • Work closely with the Primary Care Network to expand the role and team.
    • Support practices in managing care home patients and adjusting Personalised Care & Support Plans.
    • Assist care home patients with anticipatory care discussions and planning.
    • Administer acute medical treatment for care home patients.
    • Assess and treat patients with various health care needs.
    • Prescribe and review medications in alignment with guidelines.
    • Order investigations and manage results appropriately.
    • Work with a multidisciplinary team to enhance care for care home patients.
    • Adopt a personalized and population-centered care approach.
    • Support practices with ward rounds and MDTs.

    Pontefract & Knottingley PCN is composed of four GP practices serving a population of around 48,000 registered patients. Our goal is to establish a team of professionals who collaborate with volunteer and community services to deliver top-notch services to our local community.

    Qualifications:

    Essential

    • Working towards a Post Graduate Degree Qualification in Clinical Practice or equivalent.
    • Registered Healthcare Practitioner.
    • Independent prescribing qualification.

    Desirable

    • Completed Post Graduate Degree Qualification in Clinical Practice or equivalent.
    • Teaching/mentorship qualification.

    Knowledge:

    Essential

    • Advanced clinical practice.
    • Management of patients with complex needs.
    • Local and national health policy understanding.
    • Clinical governance comprehension.
    • Knowledge of Primary Care Environment.

    Desirable

    • Understanding of palliative care and care homes.
    • Familiarity with public health issues and determinants of health.
    • Management of long-term conditions.
    • Proficiency in SystmOne/EMIS.

    Skills:

    Essential

    • Clinical leadership.
    • Effective communication skills.
    • Conflict management and negotiation.
    • Resource management.

    Desirable

    • Service development.

    Experience:

    Essential

    • Proven post-registration experience.
    • Managing minor conditions.
    • Developing protocols and guidelines.
    • Leadership in clinical benchmarking.
    • Team leadership.
    • Auditing.
    • Teaching and mentorship.
    • Telephone triage and consultations.
    • Experience in primary and community settings.

    Desirable

    • Project management.
    • Community development initiatives.
    • Health-needs assessment.
    • Working with care homes and palliative care.
