    Wentworth 1 Primary Care Network is recruiting a number of AdvancedNurse Practitioners to work within our Acute Access Hubs. This is an exciting opportunity to join our team where same day access for acute patients is accessed via a PCN hub. This role will serve our Rawmarsh Hub, serving 3 small practices. The role is 3 full days per week.

    The Advanced Nurse Practitioner will utilise a range of advanced skillsand expert knowledge for patients presenting with undifferentiated andundiagnosed conditions. The post holders will demonstrate critical thinking inthe clinical decision-making process. Clinical care will include new patientclerking, initial history taking, face-to-face assessment, examination,diagnosis, treatment, management, referral, and evaluation of care.

    Main duties of the job

  • Work autonomously and be accountable for his/her own professionalactions.
  • Undertake clinical decisions at an advanced level using expert knowledgeand clinical skills to deliver holistic care to people accessing primary careservice.
  • Assess undifferentiated patients with a range of acute, non-urgent andchronic medical decisions.
  • Obtain a clinical history, undertake appropriate physical examinations,and investigations, formulate differential diagnosis and carry throughmanagement treatment plans ensuring relevant follow up.
  • Undertake same day acute clinics either through face to face consultations, telephone or video consultation.
  • Working as part of a larger primary care network to improve patient careand ensure more efficient ways of working.
  • Promote a learning environment for patients, nurses and other healthprofessionals.

    Wentworth 1 Primary Care Network includes 7 member practices. We provide primary care for over 52,000 patients in the Rotherham area.

    Our aim by working together to co-ordinate, plan and share resources is that we will develop a stronger, more resilient primary care offer for all our patients.

    This new way of working brings together partners from across the health, social care and voluntary sector to design and deliver innovation in services better placed to meet the needs of our communities, ensure better health outcomes for patients and utilise resources in a more efficient way. This includes the creation of additional roles in our practices including Clinical Pharmacists, Social Prescribers and Care Coordinators.

    Job responsibilities

  • Maintain their professional registration. Work within the mostup to date relevant professional standards set by individual professiongoverning body.
  • Record accurate consultation data in patients records in theclinical system, all practices use Systm1, in accordance with the up-to-date professionalstandards
  • Make effective use of the clinical system by ensuring consistentand accurate data entry and use of read codes.
  • Instigate necessary invasive and non-invasive diagnostic testsor investigations and interpret findings/reports to discuss with clinician asappropriate.
  • As an independent prescriber, the post holder will prescribesafe, effective and appropriate medication as defined by current legislativeframework within scope of practice.
  • Provide safe, evidence-based, cost-effective, individualisedpatient care
  • Offer a holistic service to patients and their families,developing where appropriate an on-going plan of care/support, with an emphasison prevention and self-care and support patients with Socio-economicissues.
  • Promote health and well-being through, health education,screening and therapeutic communication skills to improve self-care and promoteindependence for patients.
  • Participate in screening patients for disease risk factors andearly signs of disease.
  • Refer patients directly to other services/agencies asappropriate or after discussion with a GP.
  • Encouraging patients to rely on face-to-face consultations onlywhere there is clinical need to.
  • Working with allied health professional to ensure Quality andOutcome Frameworks are delivered
  • Under complex decision making which involves sensitive orcontentious information and occasionally deal with situations which are hostileor emotive.
  • Exercise professional judgment to manage risk appropriatelyespecially where there maybe complex and unpredictable events and supportingteams to do likewise to ensure safety of the individuals families and carers.
  • Use enhanced communication, negotiation, and conflict managementskills and demonstrate empathy, reassurance and adopt persuasive skills toovercome barriers to acceptance.
  • Work with multi-disciplinary team within the Practice, andacross the wider primary care network to promote integrated and seamlesspathways of care
  • Monitor the effectiveness of their own clinical practice throughthe quality assurance strategies such as the use of audit and peer review

    Experience

    Essential

  • Evidence of both team and autonomous working.
  • Can show evidence of using autonomous physical examination and assessment skills.
  • Can demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children.
  • Consolidated Independent Prescribing experience.
  • Use of examination skills in practice.
  • Experience of telephone triage.
  • Desirable

  • Experience of working in Primary Care.
  • Qualifications

    Essential

  • Registered Adult Nurse registered with the Nursing and Midwifery
  • Council (NMC) or, Paramedic registered with the Health Care Professions
  • Council (HCPC)
  • Registered as an independent nonmedical prescriber is a must.
  • A minimum of 2 modules at Level 7. One module must have included assessment of clinical examination skills
  • Desirable
