    Job summary

    Anexciting opportunity has arisen to join our dynamic forward thinking CommunityCardiac Rehabilitation Service as part of the Community Cardiac team. We arelooking for an enthusiastic Nurse with Cardiac Rehabilitation experience andknowledge to help us develop our existing provision. This is a band 6 post butwe will consider a band 5 as a developmental opportunity.

    Main duties of the job

    Youwill work closely alongside the skill mixed team in delivering the BACPRstandards and core components of cardiac rehabilitation; this may be inCommunity or Hospital. If appointed as a B5 developmental post you willwork to complete the BACPR competencies and towards fulfilling the requirementsof the B6 job description and person specification. Patients will have apersonalised assessment tailored to their condition and needs and then agree ahybrid Cardiac Rehabilitation programme from a menu based set of options. Youwill work with Patients/Family/Carers to identify their risk factors and setgoals. A passion for empowering Patients in reaching their goals and usemotivational interviewing is important to facilitate this. You will support theimplementation of the Cardiac Rehabilitation plan and assess Patients progressregularly along the pathway. You will encourage engagement with all aspects ofrehabilitation. Identifying symptoms and responding to these and escalating asappropriate is also required. A key role is in educating Patients and Carersabout early symptom recognition and healthy lifestyle. You will support thedelivery of exercise sessions and assess Patients face to face, virtually, ineither groups or one to one settings, within the community or Patient homes.You will support in meeting the teams KPIS, the ethos and vision of the serviceand core components of Cardiac Rehabilitation and liaise back with the CardiacRehabilitation Specialist Nurses.

    Person Specification

    Experience

    Essential

  • Extensive post registration experience with subsequent cardiac experience and understanding of cardiac conditions
  • Experience in delivery of cardiac rehabilitation
  • Experience delivering community based cardiac care
  • Evidence of on-going professional development
  • Evidence of proactive involvement in the teaching and mentorship and development of nurses and nursing practice

    • Desirable

  • Evidence of involvement in the development of protocols and clinical audit

    • knowledge

    Essential

  • Knowledge of local policies, practices and procedures within specialist area and a good understanding of national strategic context of the NHS and knowledge of NICE and BACPR standards and core components

  • First degree in nursing or an equivalent portfolio of evidence
  • Teaching/assessing qualification

    • Desirable

  • Non-medical prescriber or willingness to work towards
  • Advanced Physical assessment and consultation skills or willingness to undertake
  • Post basic qualification/ course in cardiac related subject
  • BACPR course or Heart Manual Facilitation Course Training
  • Specialist training relevant to role (or willingness to undertake)
  • Cannulation/venepuncture ECG and interpretation
