We are seeking a Band 4 Community Nurse Associate to join the Out of Hours Countywide community nursing team. As a nursing associate, your role involves providing high-quality nursing care to patients in their own homes, working in collaboration with a multi-disciplinary and neighbourhood team. The focus is on ensuring patient-centered care with active involvement in decision-making.
Our service operates seven days a week, engaging closely with other community services and PCN's to deliver top-notch care. This role offers ample opportunities for personal development and learning.

Job Responsibilities:
