    Job Title: Band 4 Community Nurse Associate

    Company: Cambridgeshire and Peterborough NHS Foundation Trust

    We are seeking a Band 4 Community Nurse Associate to join the Out of Hours Countywide community nursing team. As a nursing associate, your role involves providing high-quality nursing care to patients in their own homes, working in collaboration with a multi-disciplinary and neighbourhood team. The focus is on ensuring patient-centered care with active involvement in decision-making.

    Our service operates seven days a week, engaging closely with other community services and PCN's to deliver top-notch care. This role offers ample opportunities for personal development and learning.

    Main Duties of the Job:

    • Experience in community nursing, palliative care, and diabetes patient care
    • Educated to Registered Nursing Associate status on the NMC Register
    • Familiarity with falls prevention or willingness to acquire this skill
    • Proficiency in caring for adults and older individuals
    • Ability to independently assess patients and create care plans
    • Demonstrate a commitment to high-quality service delivery
    • Flexibility to work both independently and as part of a team
    • Good time management and workload organization skills
    • Strong communication and interpersonal abilities

    Job Responsibilities:

    As a member of the Community Nursing Team, you will perform delegated duties to meet the service users' needs. You will receive training and supervision from registered professionals to deliver care in patients' homes.

    This position provides an excellent opportunity for motivated individuals to contribute to our service's mission of providing high-quality care in the community.

    To be accountable for all your delegated work and follow professional guidelines. You will be responsible for managing your time efficiently, communicating effectively with patients and colleagues, and maintaining accurate records.

    Person Specification

    Education Criteria

    Essential:

    • Registered Nursing Associate on the NMC register
    • Minimum Level 5 Foundation Degree in Nursing Associate Practice
    • GCSE's Grade A - C in Maths and English or equivalent

    Desirable:

    • QCF in Care / Health & Social Care Level 2/3 or equivalent
    • Completed Care Certificate

    Experience

    Essential:

    • Experience in in-patient services and multi-disciplinary teamwork
    • Basic understanding of complex health needs

    Desirable:

    • Experience in providing supervision to junior colleagues
    • Skills & Abilities

      Essential:

      • Strong interpersonal and communication skills
      • Appropriate IT skills
      • Ability to work effectively in a team
      • Initiative-driven work approach
      • Participation in reflective practice and supervision activities
      • Knowing when to seek advice from health professionals

      Desirable:

      • Decision-making, delegation, and problems solving skills
