Personal Statement 

I consider myself to be hardworking, conscientious and work well as part of a team as well as independently. I am keen to learn and enjoy learning new skills. I am passionate about putting the service user at the heart of all the work I do and advocating on their behalf. 

Employment History 

November 2022 to date

I have been working as a personal assistant to a young chemistry student (C).  This role involved a 12 hour shift pattern.  C has an undiagnosed neurological disorder which presents as paralysis to her lower body.  She is a wheelchair user as a result.  C also has a diagnosis of  autism.

My role was to assist C to get washed and dressed, support to University, assist in the preparation of meals and provide emotional and psychological support.

July 2020 – October 2022 

I had taken a career break during this time due to personal circumstances including a house move. During this time I have undertaken an online course (Awareness of Mental Health) with Distance Learning at Scarborough TEC which has been successfully completed. The qualification is TQUK Level 2. I am currently undertaking a further course (Counselling Skills).

Fareham Borough Council – January 2020 – July 2020 

I worked with this Adult Community Team as a locum social worker. The position involved carrying out assessments in people’s homes in order to provide service users with the support that they needed. The work included carrying out Mental Capacity Assessments and being involved with other professionals in Best Interest Assessments. 

Hampshire County Council – June 2019 – December 2019 

I worked with the NHS Funded Nursing Care Team carrying out assessments for Continuing Health Care funding. The assessment comes in two parts; an initial assessment or checklist which informs whether the service user meets criteria for NHS Continuing Health Care. The checklist is followed by a more comprehensive multi-agency approach to gain deeper insight into the service users needs. The work involved working closely with health colleagues, service providers and family members. 

Great Western Hospital, Swindon – June 2018 – May 2019 

This role involved assessments of patients for discharge into the community either to home or ongoing care. I worked closely with patients, their families and other agencies to ensure safe 

discharge of patients. The role involved assessment for discharge, mental capacity assessments in some cases and where there was a lack of capacity I chaired Best Interest meetings. 

Southampton City Council - April 2017 - December 2017 

I worked with the Single Point of Access team (SPA) which is a non-visiting team. My role in this team was to triage safeguarding referrals. I worked closely with other agencies including the police, hospital staff, other local authorities and trading standards to ensure referrals were dealt with appropriately and in a timely manner. The work included making emergency referrals for respite care and seeking funding for the same. 

Royal Bournemouth Hospital - January 2017 - April 2017 

I worked with the Interim Care Team as a locum social worker. The team is dedicated to sourcing interim beds to avoid the negative consequences of a prolonged hospital stay. The role involved close liaison with patients, their familit=es and the interim bed providers to support a safe and effective transition out of hospital. 

Hampshire County Council - August 2015 - 12/2016 

I worked initially with the duty team, a fast paced environment dealing with out of hours matters, safeguarding concerns, referrals from MASH (safeguarding hub) and financial queries. 

I was asked to move to a community team in Alton where I held a complex case load involving hospital discharges, Court of Protection matters, deprivation of liberty and section 117 discharges. 

Portsmouth City Council - September 2010 - July 2015 

I had many social work roles during my time with the Council. I worked initially with the Learning Disability Team, then a review team and for over three years I worked with a dedicated safeguarding team. 

During my time with the Council I carried out social care assessments, mental capacity assessments, Human Rights Act assessments and carers assessments. I supported service users to ensure their safety and independence. 

I worked closely with a range of teams including health, housing, police and social care professionals in order to construct and implement risk management plans that provide professional support to victims and perpetrators of domestic abuse and violent sexual abuse. My work with the safeguarding team included investigating a number of complex cases involving domestic violence, honour based violence, assessment under s.21 National Assistance Act and the Human Rights Act. 

Portsmouth City Council -09/2003 - 04/2007 

Prior to embarking on my degree course I worked as a Shared Lives Carer for a gentleman with learning disabilities who lived in our home. I worked closely with Social Services to ensure the place met the needs of the gentleman and his family. The role involved personal care, risk assessment, review of financial entitlements and advocating on his behalf. 

Previous experiences 

I have a background in law, nursing and teaching exercise. I have lived abroad in Dubai, Italy and Portugal for a total of three years.
