About Evelyn Hartigan :

A highly motivated newly graduated Social Worker with significant and progressive experience indeveloping person centered care plans and excellent assessment for vulnerable adults with physical and mental impairment in hospital discharge team, wellbeing community team, voluntary sector and drug and alcohol service and adults with learning. 

My Experience as a social worker, social work assistant, on placement and as a support worker/health care assistant and live-in-care has developed my skills, knowledge and confidence in writing excellent assessments and person centered care plans that represents each individual service user’s needs. My experience also improved my ability to advocate for service users during crisis and worked in partnership with service users and their family, colleagues, other agency to achieve service user’s desired outcome.

Experience

    Ability to undertake assessment and care management responsibilities within the team and ability to facilitate effective, safe and timely discharges from hospital. Ability to contribute to the overall development of appropriate service provision within the organization.

• Ability to maintain a professional record on each service user and to write clear, concise reports and case summaries when required. Able to ensure records are incorporated into MDT case file and ability to commit to client centered and non-discriminatory practice

• Ability to offer 1:1 contact with service users on a weekly basis to ensure excellent assessment and to facilitate safe discharge.

• Experience of completing Standard Assessment Procedure(SAP), Living well assessments, first contact assessment, Care Act Care and Support Plan and service delivery plan that is tailored to service user’s needs. Experience of attending and participating in Multi-Disciplinary Team meetings. Highly developed interpersonal skills

• Experience of working under pressure in collaboration with other agencies. Ability to prioritize, and produce safe, timely and effective discharge.

• Experience of participating and contributing during team meetings and case discussing.

• Excellent communication skills. Ability to deal with conflict and to maintain effective working relationships with relevant parties.

• An individual who is caring, empathetic, enthusiastic, flexible, motivated, innovative, ability to use own initiative and keen to learn

Education

Bachelors of Art Social Work (Attended London South Bank University)

Advanced Diploma Health and Social Care (Attended Newham College of Further Education)
