Scheme Manager

London - Royal London Whitechapel

Monday-Friday (40 hours per week, between the hours of 8am-6pm)

Salary:
£28,000 - £33,000 per annum (D.O.E)

We are looking for an organised and professional individual to work closely with hospital discharge teams and care providers to source the best care for our clients.

This is an extremely rewarding role as you will be working towards a quick and safe dischargeplan, helping families often during great times of stress whilst motivating your team to achieve all of their goals.

**Every day you will be:
- *
  • Managing a small team who support families and individuals to find care either in their own home or within a care home
  • Liaising with healthcare professionals, families, clients and care providers throughout the process of sourcing care.
  • Ensuring you do everything in your power to get the best possible package of care for each client
  • Building strong relationships with healthcare colleagues, commissioners and local care providers.
  • Travelling to different locations discharge hubs, wards, care homes as required
  • Monitoring reporting and KPI tracking for the scheme

**To thrive in this role you must ideally have: - **- The ability to work within a fast paced/challenging environment with constant multitasking, delivering a high standard of customer care.

  • Understanding of Healthcare specifically hospital and community structures would be highly beneficial
  • A skillset to coach, support and reward a team
  • Strong interpersonal skills an able to communicate well with all departments, including patients and a range of professionals on sensitive matters.
  • An organised approach to work with effective planning skills and be able to think proactively on your feet
