    An exciting opportunity has arisen to join the Whittington Health NHS Trust's Children's Continuing Care Team as a Band 3 Support Worker.

    The Continuing Care Team provides care to children or young people, who are in receipt of a Continuing Care package within the London Boroughs of Islington, Camden and Haringey.

    The successful applicant will join a dynamic team of Children's Nurses and other Community Support Workers who provide shifts for children and young people in their homes or educational community settings. These children and young people have complex health care needs. You will also have the opportunity to work in the paediatric acute setting.

    You will be a highly motivated and enthusiastic individual with a willingness to complete a robust and in-depth training programme which is tailored around the health needs of the child or young person. You will receive ongoing training and support throughout your time with the team.

    This is a full-time post. Shift patterns are predominantly night shifts with some daytime activities with team days and training. The vacancy is a fixed term period of 12 month.

    Main duties of the job

    To work as a member of a team to carry out delegated tasks, to provide support to families of children with complex care needs with regular support from the children's nurses in the continuing care team.
    To facilitate the delivery of therapy programmes
    To ensure the children in your care are your first concern, treating them as individuals and respecting their dignity.
    Work with families to promote the child's wellbeing.
    Provide a high standard of care at all times while on shift meeting the individual needs of the child or young person.

    To maintain up-to-date, accurate records, in-put work data into the computer and to provide written reports as requested.
    To participate in Rio, Care Flow training and ensure that IT systems are used appropriately.
    To report accidents and incidents in accordance with health & safety policies.
    To maintain an up to date record of continuous professional development.
    To be responsible for ensuring an accurate and up to date annual leave record is available.

    To participate in the User Audit process.
    To ensure that the privacy and dignity of children, young people and their families is maintained at all times.
    All information concerning patients/clients and staff must be treated as strictly confidential at all times.
    To ensure that children, young people and their families are included in the decision making process.

    Detailed job description and main responsibilities

    Clinical
    To provide and assist with the nursing care of sick children and young people in their homes and elsewhere in the community.
    To be responsible to the Senior Practitioner for all aspects of their work.
    To work the hours allocated to the child or young person as a result of following the Continuing care framework process.
    To participate in the assessment of the needs of sick children & young people and families, who have complex nursing & medical needs.
    To accurately observe and record any changes in child and family and initiate appropriate action as required.
    To ensure a safe environment and the safe use of equipment.
    Employees must be aware of the responsibilities placed on them under the Health and Safety at Work Act (1974), and to ensure that agreed safety procedures are carried out to maintain a safe environment for employees, patients and visitors.
    Be responsible for behaving in a manner that does not put the child, family, other team members or themselves in danger.
    Behave in such a way that when at work, you are able to maintain a high level of alertness, to ensure the patient's needs are fully met.
