    Job summary

    As part of the NHS Long Term Plan, we are expanding nursing services to our practice population. An exciting opportunity has become available for a Registered Nurse with advanced clinical skills and chronic disease management, specifically diabetes, to join our experienced and enthusiastic team of nurses within a large, busy GP practice. We are looking for someone who is motivated and enjoys working as part of a team. We have a vibrant multicultural practice population and work closely with the wider multidisciplinary teams.

    Main duties of the job

    You will need to have had experience in minor illness disease management and in autonomously managing diabetes in a community setting, but you will work alongside Advanced Nurse Practitioners with many years of working in these fields. Non-Medical Prescribing qualification would be desirable

    Essential

  • Registered Nurse with the Nursing & Midwifery Council (NMC)
  • Hold an Advanced Nurse Practitioner qualification (or equivalent)

    • Desirable

  • Qualified independent nurse prescriber
  • Qualified triage nurse

    • Experience

  • Experience of working in a primary care environment
  • Have advanced clinical skills
