This post provides an exciting opportunity for a qualified and experienced mental health nurse, social worker or occupational therapist to work within Connecting Care in Wakefield.
The primary care mental health service is expanding and is a key component of our community mental health transformation.The role will involve assessing mental health needs, delivering direct interventions with service users to address short term, low level mental health issues, signposting referrals to appropriate support and raising awareness of mental health within the workforce.
The post holder must be able to demonstrate a good understanding of the improving physical health care agenda for service users with mental health problems to promote health and well-being and prevent illness.
This post holder will work across the Waterton and Bullenshaw Connecting Care Hubs covering the whole of the Wakefield district.
Agile working is expected and the hours of work currently are across 5 days 8.The post holder will be responsible for conducting a mental health assessment with all people referred to him/her, determining what the individuals mental health needs are and the most appropriate service/ agency to meet those needs.
The post holder will build and maintain effective links with all disciplines within the 3 hubs, and will work as an integrated part of the integrated care Team .
The post holder will also provide easily accessed advice for the integrated care teams and support identification of the training needs of staff on mental health issues.
The post holder will participate in providing training and supervision, as required, for staff within the hubs on mental health issues.
The post holder will also undertake a training, consultancy and support role within the team and for the wider integrated care workforce.
