    Description

    This post provides an exciting opportunity for a qualified and experienced mental health nurse, social worker or occupational therapist to work within Connecting Care in Wakefield.

    The primary care mental health service is expanding and is a key component of our community mental health transformation.

    The role will involve assessing mental health needs, delivering direct interventions with service users to address short term, low level mental health issues, signposting referrals to appropriate support and raising awareness of mental health within the workforce.

    The post holder must be able to demonstrate a good understanding of the improving physical health care agenda for service users with mental health problems to promote health and well-being and prevent illness.

    This post holder will work across the Waterton and Bullenshaw Connecting Care Hubs covering the whole of the Wakefield district.

    Agile working is expected and the hours of work currently are across 5 days 8.The post holder will be responsible for conducting a mental health assessment with all people referred to him/her, determining what the individuals mental health needs are and the most appropriate service/ agency to meet those needs.

    The post holder will build and maintain effective links with all disciplines within the 3 hubs, and will work as an integrated part of the integrated care Team .

    The post holder will also provide easily accessed advice for the integrated care teams and support identification of the training needs of staff on mental health issues.

    The post holder will participate in providing training and supervision, as required, for staff within the hubs on mental health issues.

    The post holder will also undertake a training, consultancy and support role within the team and for the wider integrated care workforce.

    Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. P.A. procedures and risk management protocols.
    Assesses needs and identifies problems relevant to the care of people referred.

    ~ Devises a plan of care in partnership with the person and carers.

    ~ Implements the planned programme of care to ensure a high standard is achieved.

    ~ Reviews the effectiveness of the care provided and where appropriate initiates any action.

    ~ Advises and supports the person and carers to promote health and well-being and to prevent illness.

    ~ Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required.

    ~ Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community.

    ~ Operational Policy and participates in the team duty rota.

    ~ Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

    ~ Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
    Participates in the formulation of policies at unit level.
    Supervises students and trainees on placement.

    ~ Assists in the teaching and in-service training of staff.

    ~ Attends courses or training sessions relevant to the updating of knowledge and experience.

    Promotes an understanding of mental health and well being.

    ~ Ensures health and safety regulations are observed.

    ~ Attends mandatory training sessions as directed by the Trust.

    Report any concerns regarding the safety or wellbeing of children, adults service users, members of their families etc, in accordance with Trust Policy.

    Significant experience of working within a mental health setting
    Can demonstrate skills in assessing and managing people experiencing mental health problems
    Experience of supporting/mentoring and supervising colleagues/students/trainees
    Can demonstrate skills in assessing and managing risk in a variety of settings

    NICE, CQC, CQUINs

    Experience of working holistically with Adults with an emphasis on coexisting mental and physical health needs that impact on wellbeing.

    Can evidence awareness of equality and diversity and is able to incorporate this into care planning and the delivery of care.

    Knowledge and skills in specialist areas such as dual diagnosis, peri-natal mental health, working with people experiencing psychosis, working with people with a diagnosis of personality disorder
