    Job summary

    The crisis service provides integrated, high quality assessment, support and interventions in and out of hours. We work closely with the Adolescent teams and borough based clinical teams across the three boroughs, as well as with tier 4 (inpatient services) which are provided by the South London Partnership. The crisis team is multidisciplinary, and is supported by a team manager, consultant nurse, operational manager,practice development practitioners and a full time Consultant Psychiatrist.

    All clinicians within the pathway will provide both crisis assessment, follow up and interventions and will be supported via bespoke training opportunities, supervision and CPD events.

    Main duties of the job

  • To provide specialist mental health advice, consultation and signposted using an outcome based triage model, to the multi-agency network.
  • To carry out effective bio psychosocial assessments of individuals in crisis referred to the team from community mental health teams, GPs and other sources.
  • To gate keep all referrals to inpatient services to test their suitability for home treatment as an alternative to admission
  • To carry out effective risk assessment and management, seeking advice from multi- disciplinary colleagues and other teams where needed
  • To provide a range of agreed brief crisis clinical interventions to service users and their relatives in any setting, aimed at increasing the individual's psychological stability
  • To develop effective crisis plans with individuals which will improve their future resilience
  • To assess the welfare of people discharged from inpatient wards in a timely manner
  • To carry out assessments of mental capacity as required to do so.
  • To liaise collaboratively with hospital and community based mental health services, both within Oxleas and elsewhere, also with emergency services and non-statutory bodies

    Job responsibilities

    You will work closely with colleagues within the crisis team, borough CAMHS services and wider colleagues across the South London Partnership. In addition, there will be opportunity to work closely with acute trust and social care colleagues.

    This role allows colleagues to develop their skills in autonomous and multi-disciplinary/agency working. Colleagues that have worked in this role have successfully been promoted to senior practitioner roles within the team, other colleagues have successful secured training to become therapists as well as some colleagues successfully moving into managerial roles.

    The Liaison and Crisis team will receive approximately 1029 crisis referrals per annum via A&E (based on all hours 2019/2020 data) across the three boroughs. 5% of these presentations took place out of hours (20:00-08:00). Around 50% of young people presenting in crisis to A&E are currently receiving treatment from Community CAMHS. At any one time there are around 390 young people on the caseload of the three adolescent teams (tier , which work with young people who present with acute mental health needs and crises and the most significant, high risk mental health conditions. The Liaison and crisis team work collaboratively with borough adolescent and crisis pathways to support those young people known to the service who present in crisis via A&E, as well as support the most vulnerable young people through a telephone support system during the evenings and weekend.

    The crisis team works with young people for the duration of their crisis, and for existing patients, this will be done in conjunction with their existing care coordinator in the Community CAMHs team.

    The liaison and crisis team works closely with the three Oxleas community CAMHS services, to ensure a coordinated approach to care and careful transition. The team undertakes assessments for young people who present in crisis out of hours and link with the existing day crisis service for these services.

    Assessment in the team consists of individual and family assessment. The outcome from the assessment might be admission to a CAMHS in-patient unit, an individual piece of work around an episode of mental ill-health, joint work alongside the Community CAMHS Care Coordinator, or working with partner organisations to facilitate appropriate multi-agency support and intervention for the young person.

    The nature of the work of the consultant psychiatrist in the team is to provide crisis assessments and interventions, supervision and consultancy to non-medical colleagues and clinical leadership including taking a lead role in managing the interface with CAMHS and hospital departments.
