    Description

    Job Summary

    We have open positions for Community Mental Health Practitioners, also known as Care Coordinators, within our CMHTs. The roles are mainly at Sudden Resource Centre in the Rochdale West CMHT, subject to change based on demand. Applicants must be registered and experienced as RMN, Social Worker, or OT.

    • Provide mental health services to working-age adults with moderate to severe mental health issues
    • Collaborate with different disciplines for a 'needs-based' approach
    • Promote integrated service delivery
    • Develop relapse prevention plans and engage with 'difficult to engage' individuals and their families

    Main Duties of the JobThe successful candidates will work with adult service users with mental health needs, manage care for a defined caseload, conduct assessments, develop care plans, implement and evaluate care, and provide skilled interventions.

    Service/Ward/Department SummaryThe CMHT works with working-age adults with Serious Mental Illness and Complex Mental Health Difficulties, focusing on recovery and optimal functioning.

    Qualifications

    • Registered Mental Health Nurse / Occupational Therapist / Social Worker

    Experience

    • Substantial experience in relevant post-registration practice
    • Experience managing a caseload and working in a multi-disciplinary team
    • Experience with severe and complex mental health problems

    Knowledge

    • Therapeutic assessment techniques and interventions for individuals with complex mental health needs
    • Screening and assessment tools for severe mental health problems
    • Understanding of adult safeguarding procedures and legal acts
    • Comprehensive knowledge of Care Programme Approach roles and responsibilities

    Skills and Abilities

    • Able to undertake the care coordinator role
    • Perform mental health and risk assessments, make accountable decisions
    • Strong written and interpersonal communication skills
    • Record relevant care records to a professional standard
    • Coordinate and delegate work effectively
