    Job summary

    Part Time 15 hours per week

    We are looking for an experienced pharmacy Technician tojoin our established medicines team at Eston PCN. We can only accept applications fromcandidates who have completed the CPPE Primary Care Pharmacy Education Pathway.

    The successful candidate will work with the support of thePCN Medicines team the including of pharmacists, pharmacy technicians, traineepharmacists and trainee pharmacy technicians as well as the full PCN clinicalteam. The successful candidate willsupport the member GP practices to deliver medicines safety projects,structured medication reviews and medicines optimisation as set out in the NHSLong Term Plan.

    Main duties of the job

    Thepost holder is a pharmacy technician, who acts with autonomy within theirprofessional boundaries.

    Thepost holder will work as part of a multi-disciplinary team with the potentialto deliver supported medication reviews in a patient-facing role.

    Thepost holder will provide expert pharmaceutical technical support to helpsupport the implementation of the medicines optimisation work programme.

    Thepost holder will undertake audits of medicines related issues across theprimary care network - evaluate results and produce and present progressreports.

    Therole is pivotal to improving the quality of care and operational efficienciesso requires motivation and passion to deliver excellent service within generalpractice.

    We are afriendly and supportive well-established practice which has recently mergedwith Manor House Surgery in Eston.Normanby Medical Centre is located in Low Grange Health Village,Middlesbrough. The practice operatesfrom a purpose-built premise. Trainingpractice for medical students from Newcastle and Sunderland and approved GP registrartrainer from August 2021. Highly recommended as teaching practice by Medicaland Nursing students. Patient population is 22,700. Computer system is EMIS Web. (changing toSystmone -February 2024) CQC rating'Good'. High QOF achievement.

    Our teamcomprises of; - 12 GP Partners, 6 Nurse Practitioners, 10 Practice Nurses, 4Health care Assistant, 2 Pharmacist, 1 Clinical Pharmacist (PCN) and a highlyskilled and supportive administration and reception team with good staffretention.

    We are thelead employer for Eston PCN with a Partner as the PCN Clinical Director. We are supported by; - social prescribers,pharmacists, Dietician, FC-Physiotherapist, Physician Associates, a Care co-ordinator, a Mental HealthPractitioner, Podiatrist, Health and Well-being Coach, Pharmacy Technician,Admiral Nurse. We have strong workingrelationships with our practice attached community services, CCG and localpractices. We provide our own enhancedaccess on an evening and Saturday within our PCN. We are also part of ELMFederation.

    Job responsibilities

    Undertakepatient facing and patient supporting roles to ensure effective medicines use,through shared-decision making conversations with patients

    Carryout medicines optimisation tasks including effective medicine administration( checking inhaler technique), supporting medication reviews, and medicinesreconciliation

    Whererequired, utilise consultation skills to work in partnership with patients toensure they use their medicines effectively

    Support,as determined by the PCN, medication reviews and medicines reconciliation fornew care home patients and synchronising medicines for patient transfersbetween care settings and linking with local community pharmacists

    Providespecialist expertise, where competent, to address both the public health andsocial care needs of patients, including lifestyle advice, service information,and help in tackling local health inequalities

    Takea central role in the clinical aspects of shared care protocols and liaisingwith specialist pharmacists for more complex patients

    Supportinitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing

    Assistin the delivery of medicines optimisation and management incentive schemes andpatient safety audits

    Supportthe implementation of national prescribing policies and guidance within GPpractices, care homes and other primary care settings. This will be achievedthrough undertaking clinical audits ( use of antibiotics), supportingquality improvement measures and contributing to the Quality and Outcomes Frameworkand enhanced services

    Workwith the PCN multi-disciplinary team to ensure efficient medicinesoptimisation, including implementing efficient ordering and return processes,and reducing wastage;

    Supportthe practice medication team in sortingand streaming general prescription requests, so as to allow GPs and clinicalpharmacists to review the more clinically complex requests;

    Provideleadership for medicines optimisation systems across PCNs, supporting practiceswith a range of services to get the best value from medicines by encouragingand implementing Electronic Prescriptions, safe repeat prescribing systems, andtimely monitoring and management of high-risk medicines

    Providetraining and support on the legal, safe and secure handling of medicines,including the implementation of the Electronic Prescription Service (EPS)

    Developrelationships with other pharmacy technicians, pharmacists and members of themulti-disciplinary team to support integration of the pharmacy team acrosshealth and social care including primary care, community pharmacy, secondarycare, and mental health

    Person Specification

    Qualifications

    Essential

  • Must hold recognised qualification in pharmaceutical sciences (BTEC / NVQ3 or equivalent)
  • Must be a registered technician with General Pharmaceutical Council
  • Must have Statement of assessment and progression from CPPE Primary Care Pharmacy Education Pathway
  • Desirable

  • 2 years relevant post qualification experience
  • knowledge and skills

  • Able to plan and work independently and manage competing priorities
