    The post holder will work in collaboration with partner agencies both voluntary and statutory to achieve good health outcomes for children and their families.

    The Community Nursery Nurse (CNN) is an integral member of the Children's team.

    The post holder will support the Health Visitor in their work with children and families, with the overall aim of helping each identified child to achieve its full potential.

    The work of the CNN will be part of a planned programme of care within the Healthy Child programme (0 – 19) delegated by the Health Visitor, who has completed the initial assessment.

    All activities undertaken by the Nursery Nurse will be at the discretion/delegation of the Health Visitor/School Health, after appropriate in-service training has been given, and level of competency demonstrated.

    The post holder will ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.

    The post holder will be responsible and accountable to the Team Manager and other Health Visitors who will delegate work.

    Contribute to a family health needs assessment planning interventions and reviewing in agreement with the Health Visitor.
    Assist and carry out developmental health reviews in line with the Healthy Child Programme where appropriate.

    Give routine advice and information in respect to child care, development, infant feeding, breast feeding, weaning, behaviour management, play, hygiene, dental care, accident prevention, attachment, parenting skills and local community facilities.

    Provide care in a variety of settings including clients' home, Children Centres, G.P. surgery, health centre or other local community centre.
    Participate in Public Health work / community development activities in line with the Public Health Agenda eg. Posters, Health and Safety days.

    Work collaboratively as a team member to achieve team objectives in the promotion of health and wellbeing of children aged 0-19 years and their families.

    Work within local and national child safeguarding procedures and have an occasional requirement to attend conference/core groups with the Health Visitor.

    The post holder is responsible for managing own allocation workload supervised by and accountable to Team Manager and/or Health Visitor/School Health.

    Be responsible for organising their own work assessing and planning whilst considering priorities including home visits and parenting groups.

    Implement and evaluate individual programmes of care as agreed with Health Visitors.

    Be responsible for updating and informing the Health Visitors on the progress care or any significant/relevant changes related to children and families.

    The post holder will work within recognised departmental policies and procedures.

    EDUCATION AND DEVELOPMENT

    Continually maintain and improve professional knowledge and competence through reflective practice, formal/informal education and the application of evidence based practice.

    Undertake educational courses, mandatory training and training as identified by appraisal and Children and Families service objectives.
    Deliver group health promotion sessions in a variety of settings, e.g. parenting groups, mother and toddlers in agreement with the Health Visitor.
    Work within and implement Clinical governance guidelines, policies, protocols and procedures.
    Actively contribute to the appraisal process, identifying professional objectives and personal development plans.
    Engage in clinical supervision & management supervision as per organisation policy.

    Identify and report any accidents / incidents in appropriate documentation in accordance with organisational policy and take remedial action as appropriate.

    Manage complaints from clients and colleagues as per organisation policy. Manage risk, health and safety and work within policies and procedures laid down by the organisation.

    Lone-worker for majority of client interactions and as such will be expected to work within agreed Lone-worker Policy / Guidelines.

    Participate in audits and practice development projects within the team and Trust wide.
    Contribute to the collection and maintenance of evidence for Care Quality Commission (CQC) outcomes.
    Proactively establish good communication networks with clients, families, carers, members of statutory and voluntary agencies and colleagues.

    Communicate childcare and child centred information with empathy and reassurance to assist parents in improving language and disability barriers with their children.

    Occasionally work in environments that contain emotive and distressing situations e.g. aggressive clients, families where children have recently diagnosed disabilities.

    Occasionally be subject to difficult and distressing family situations in particular where there is exposure to child protection, family breakdown, mental health, behavioural problems with babies and young children.
