    Job summary

    The multidisciplinary team at Leeds Vascular institute are seeking a motivated and enthusiastic Advanced Clinical Practitioner to further develop and deliver their Perioperative Frailty Service.

    This dedicated service manages vascular patients 65yo and older in line with CPOC (Centre for Perioperative Care) guidelines. Based on Comprehensive Geriatric Assessment (CGA), frailty syndromes are identified and managed to optimise patients to their best possible condition for surgery. The service offers broad and holistic care to this patient cohort, including enhanced shared decision making and advance care planning, and managing conditions when the surgical risk may simply be too high. It prides itself on full integration and collaboration with the vascular surgical and broader multidisciplinary team to deliver high quality individualised patient care.

    Main duties of the job

    The role itself involves the assessment, and management of frail older patients who are being considered for vascular surgery, including frailty assessment, management and optimisation of co-morbidities in the perioperative period, contribution to truly collaborative ward rounds undertaken by the Surgeon and the Geriatrician together, and attending ward MDT meetings. The post holder will be in the privileged position of participating in enhanced shared decision making and advance care planning and planning the consequential care. The role has a significant outpatient component, working alongside Surgeons and Anaesthetists to assess and optimise care prior to any proposed admission/procedure. Additional support and training will be provided if this is unfamiliar clinical territory.

    This autonomous role has excellent potential for personal and professional, as well as service development and leadership opportunities. The job is Monday-Friday (8-4) although there is some scope for flexibility here, and applications from people wishing to work LTFT will be considered.

    The successful applicant will actively engage in audit and clinical governance activity and the current post holder has also presented work at national conferences.

    There is scope to engage with other surgical specialties across the Trust, promoting the work of frailty services in the older surgical patient.

    It is hoped that the service will further expand in the coming year and the incoming post-holder will have the opportunity to be at the forefront of new developments including supervising and training additional ACP/tACP team members.

    Job responsibilities

    The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients within the Leeds Vascular Services. The ACP is accountable for the total episode of care for patients.

    The post holder will act as a role model, delivering advanced practice and promoting clinical effectiveness in care. He / she / they will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients.

    The post holder will work closely with all members of the multidisciplinary team and will play an integral role in the operational/strategic development of the service.

    The post holder will promote and facilitate evidence based practice and clinical audit and research within the Trust.

    The job holder will work as an autonomous practitioner within the Vascular Ward, delivering all aspects of management and care to a wide range of patients with varying needs.

    Qualifications

    Essential

  • Registered with relevant professional body
  • Degree-level registration qualification or equivalent post-registration award
  • MSc in Advanced Practice (or PG Diploma in advanced practice for those with other Masters qualification)
  • Life Support qualification
  • Relevant teaching qualification.
  • Non-medical prescribing
  • Desirable

  • PG certificate in education
  • Band 7 experience.
  • Experience of working within speciality.
  • Good Clinical Practice Certificate
