    Description
    We are looking for full time enthusiastic team players to come and join our nursing team. Evesham Community Hospital is a 50 bedded stroke and general rehabilitation unit.

    The nursing staff work alongside the therapy team and adopt a multidisciplinary team approach to providing rehab to the patients.

    We require an innovative, enthusiastic, motivated individual interested in all areas of rehab.

    You will be required to plan, implement, evaluate and take responsibility for the nursing care of the ward patients on a daily basis.

    Evesham Community Hospital plays a critical role in the delivery of local NHS services, providing more care and treatment closer to peoples homes.

    Emphasis is to reduce length of stay whilst ensuring excellent care which is safe, effective, personalised and compassionate and to the standards you would expect for your family and friends.

    As a Staff Nurse you will be responsible for providing care to people admitted to our wards for rehabilitation.

    Our focusispromoting the comfort and wellbeing of patients, enablingthemto live as independently as possible.

    Our nurses are required to facilitate the day to day running of the ward, administering medications, working alongside all members of the MDT as well as patients, their families and carers to provided 24/7 rehabilitation to enable the highest level of independence and quality of life.

    Our nurses are required to attend meetings as needed such as MDT and CHC or Discharge Planning.

    Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients.

    Responsible for the assessment, planning, implementation and evaluation of patients care within the Ward/unit, working in accordance with the philosophy of patient centered care and to NSF and other Trust standards.

    Practice as a member of the Community Hospital Nursing team, to support and work in partnership with the wider multidisciplinary team.

    Planning and Delivery of Individual Nursing Care under Direction of Senior Staff
    Work within the NMC Code of Professional Conduct.
    Assess, plan, and, implement and evaluate patient focused nursing care.
    Document care given and maintain contemporaneous records.
    Maintain records in accordance with the Trusts policy and that of NMC Code of Professional Conduct.
    Assist in implementation of health promotion strategies, as agreed by Ward Manager.
    In consultation with senior qualified staff, provide and monitor patient centered care within Trust agreed standards.
    Carry out delegated nursing care.
    Use and participate in research / audit in care delivery.
    Adhere to and promote all clinical practice, health and safety and corporate Trust Policies and Guidelines.
    Experience gained during pre-registration training

    NMC Registered
    Accredited courses in Palliative Care, Tissue Viability, Diabetes, Dementia
    Ability to communicate effectively in English in both verbal and written formats
    Able to carry out physical requirements of the role e.g. using mechanical hoists to move patients
