    Job summary

    We are excited to be recruiting Band 4 practitioners to work in our Mental Health Access Service at St Mary's Hospital. Working in Portsmouth offers a diverse inner-city client base. We want you to come and join us helping our service users connect to the most suitable service for their mental health needs.

    We are expanding our team to provide a new service for Portsmouth residents which will enable quick access to a range of services for expert assessment and interventions. This includes a telephone line that residents can access, our team of call handlers will ensure we connect the service user to a suitable pathway linking in with a variety of other agencies.

    Main duties of the job

  • To demonstrate an empathic understanding of mental health problems and issues.
  • To actively listen and autonomously help the service user to initiate a care plan, providing them with recommendations of services or linking them in with the appropriate teams.
  • To liaise with other agencies including Mental Health Teams with regard to the transfer of care to the appropriate agency.
  • To identify and provide the necessary support/advice to patients on practical issues relating to living skills such as nutritional management, self-care, social skills, benefits, housing, education, training and occupational activities.
  • To respond to the needs of people sensitively with regard to their age, race, culture, gender, religion, sexuality and disability.
  • To demonstrate knowledge of the term "professional boundaries" and apply this to working with patients, carers, and significant others.
  • To actively participate in professional development through development reviews.
  • To ensure that all incidents & complaints are reported to the team manager in accordance to trust policy.
  • To ensure we respond to the needs of people in an honest, non-judgemental, and open manner, which respects the right of individuals and groups.
  • To initiate and build therapeutic relationships.
  • To recognise scope of own abilities and responsibilities and practice within them, referring matters to and seeking advice from senior trained team members as appropriate.

    We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

    Qualifications

  • NVQ 3 or Health Related Qualification or equivalent experience
  • Experience

    Essential

  • Experience with working with people and patients, carers
  • Experience of working within a multidisciplinary team
  • To have a working knowledge of Risk Assessment and management tools currently in used within acute mental health settings
  • Desirable

  • Experience in an acute / crisis mental health setting
  • Additional Criteria

  • Is able to work legally in the UK

  • Ability to travel daily for meetings across a range of sites in Southampton/Portsmouth/Hampshire.
  • For posts which require a professional registration, postholders hold a valid up to date registration with their professional body.
  • For posts where postholders are required to drive as part of their role they hold a valid full driving licence which enables them to drive in the UK
