    Neighbourhood Nurse Clinical Nurse Lead - London, United Kingdom - Guy's and St Thomas' NHS Foundation Trust

    We are looking for an enthusiastic and clinically experienced senior nurse who can demonstrate sound clinical skills and leadership acumen to effectively manage and lead our diverse group of staff group. The post holder will be required to be flexible and work across the Boroughs of Lambeth and Southwark as needed.

    You must have experience in community nursing and in staff management.

    This role combines clinical care with leadership and management and is ideal for anyone wanting to climb the professional ladder without losing clinical skills. The role is key to providing assurance of safe standards of practice within our service.

    This is an exciting time to join our teams as we work within the Neighbourhood Nursing model. You will be working within an experienced and dynamic team and will be well supported in your role and professional growth.

    The successful applicant will be able to take a management coaching approach to support the teams. You will be required to demonstrate clinical expertise, including complex patient management. You will be trained, supported and given opportunities to develop in your career.

    Main duties of the job You will be responsible for the day to day operational management and clinical leadership of their area of responsibility.

    You will also be responsible for the assessment, delivery and evaluation of care provided to patients, ensuring all referrals are assessed by a senior nurse and that care is planned and delivered holistically.

    You will undertake advanced nursing assessment and interventions, prescribing, care and treatment including anticipatory care, complex case management and promoting health and self-management for patients.

    Through strong clinical leadership, you will supervise and support the Neighbourhood Nursing team to promote excellence in clinical care, ensuring all patients are treated with compassion, dignity and respect.

    You will promote best practices in the safe delivery of clinical care in patients' homes and the highest standards in the prevention and control of infection.

    You will inspire patient and public confidence by ensuring that the patient experience is of highest quality.

    You will also establish effective relationships with local GP's and other key stakeholders.     To co-ordinate and ensure the smooth running of all aspects of the clinical functioning of the team. Accountable for clinical and quality standards and KPIs delivered by the team. To undertake high standards of nursing care including assessment and review, care planning, implementation, evaluation, and advanced assessment and prescribing skills. To review and advise on complex patients and complete risk assessments and onward referrals as needed. To support the active management of patients with long term conditions and work towards preventing complications to reduce the need for hospital admissions. To participate in multidisciplinary working, involving other agencies as appropriate. To maintain good working relationships with other health care professionals, statutory and voluntary organisations. To utilise links with other agencies and service to facilitate appropriate care packages and provide information for clients that focus on promoting independence, self-care and wellness and keep people out of hospital. To work in collaboration with own team and others to ensure all patients that require care are supported appropriately. To undertake planned and opportunistic health promotion and education activities with patients, carers, family and neighbours, making every contact count. Maintains regular communication with GPs and other members of the Primary health care team to promote effective coordination of care including attendance at practice meetings. To take responsibility for evaluating and reflecting on his/her own practice. To perform and maintain competence in a wide variety of clinical skills to meet the needs of the service. To work collaboratively with allied agencies in meeting the needs of clients with long term illnesses. To participate and provide professional and clinical supervision.

    Education and Development

    To lead on designing learning programmes and practice experience for post registration students including Specialist Practice District Nursing. To take a lead on practice assessment and final sign off for Post registration students (including specialist practice students). To plan and facilitate learning programmes for students on community placements. To participate in and contribute to the work of the wider education team (for example in Welcome days, specific teaching sessions). To facilitate the learning of other members of the team. To champion and actively participate in organisational learning and organisational change. To take responsibility for your own professional development. To participate in clinical supervision in accordance with community services within GSTT. Practice Assessor / Supervisor to students and junior staff. Ensure that own mandatory training and PDR are up to date. To undertake performance development review of team members.

    Service Development / leadership

    To work across the organisation to support and promote Neighbourhood Nursing. To support continuous improvement and research through clinical and management activity.

    To promote research and implement change as well as quality improvement projects in the teams. To undertake investigations as needed into patient complaints, quality alerts, clinical standard concerns, rapid reviews and learning reviews. Participate in recruitment and onboarding of new staff. Informal performance management and management of behaviours. Participation in and contribution to formal HR cases. Monitoring and managing sickness absence appropriately. To ensure Health and Safety and Risk requirements are met.

    • Well developed patient assessment skills and an expert clinician
    • Ability to provide evidence-based care and decision making responsive to the needs of patients
    • Able to work as an autonomous practitioner and to make sound clinical judgements and decisions
    • Excellent organisational and time management skills and ability to facilitate these skills in others
    • Experience and ability to communicate effectively with a variety of partners and stakeholdersverbally, in writing and electronically.
    • Able to manage staff, conflict and difficult situations and to challenge and support others appropriately to positive results

    • Full UK Driving Licence
    Experience Essential
    • Significant experience working at a band 6 nurse level in a role with staff management experience
    • Experience of successfully leading change management
    • Experience of and ability to identify the development needs of others and give effective feedback
    • Experience of and ability to teach and assess students and those post qualification
    • Experience of clinical audit, benchmarking and evaluation of services
    Desirable
    • Experience as a clinical supervisor
    • Experience of working in or with primary and community care
    Knowledge Essential
    • RN (General Adult) Education to 1st degree level or equivalent.
    • Evidence of post registration education relating to the care of older people and long term conditions
    • Teaching certificate (e.g. ENB 998/997) or equivalent mentorship training
    • Short courses on various aspects of staff management
