    Please be advised that there is a temporary top up for Bands 1,2 and 3 to reflect the incorporation of the top up to the living wage of £12 per hour - £23,465 per annum. This temporary top up will be in place until the annual pay uplift for 2024/25 is confirmed

    Job overview

    An exciting opportunity has arisen for a highly motivated, experienced, passionate and dynamic nurse, providing expert leadership to the Teenage and Young Adult (TYA) Cancer across Wales, services based from the Principal Treatment Centre (PTC) for Wales, Cardiff.

    The PTC hosts a nurse led inpatient ward, day care and ambulatory care facility for TYA cancer patients undergoing chemotherapy and cellular therapy treatment. The post holder will provide expert clinical nursing leadership, support and education to all staff involved in the care of TYA cancer patients, across the network.

    Working closely with the TYA Lead Clinician, the post holder will lead the development of excellence in nursing care for TYAs with cancer. They will work alongside Network cancer leads, t hird sector organisations, specialist commissioners and local clinical teams; providing advice, service planning and strategic decision making for TYA cancer services in Wales.

    In conjunction with the Teenage Cancer Trust, they will lead service improvement in line with evidence and national policy, ensuring excellent standards.

    We are looking for a highly motivated leader who will be required to work autonomously and exercise a high level of clinical expertise when leading and advising the many teams on the specific needs of TYA cancer patients. You will need excellent communication, interpersonal skills, expert knowledge of TYA cancers and the ability to work sensitively and collaboratively with diverse teams

    Main duties of the job

    • Advance the development and practice of evidence-based TYA cancer nursing in the UHB, in line with national recommendations and measures where available
    • Collaborate effectively with all members of the multidisciplinary team in ensuring the advancement of patient focused cancer care and support
    • Work clinically on a regular basis, (this should be a least 20%) thus demonstrating expert clinical practice, professional competence, authority and credibility
    • Provide professional advice, leadership and support on TYA related haematology / oncology issues to the designated hospitals within the region
    • Is responsible for continuing management and strategic planning of the Regional TYA cancer service
    • Contribute to development of excellence in clinical care for TYA patients with cancer and their families, through working with clinical teams, providing leadership, nursing advice, service planning and strategic decision making
    • Strategically represents TYAs locally, regionally and nationally, providing expertise to planning groups for TYA cancer services and playing a key role in the development of integrated care by streamlining care across organisational boundaries
    • Work in conjunction with Teenage Cancer Trust to lead service improvement and change in line with evidence and national policy to ensure excellence in clinical care for TYAs with cancer and their families.

    Detailed job description and main responsibilities

    • Provide TYA nursing expertise and leadership to the TYA team and site specific teams with regards the complex care needs of this patient group.
    • Provide clinical leadership for the continuous professional development of the nursing staff within the Teenage Cancer Trust Unit
    • Promote Teenage Cancer Trust Units and philosophy of care surrounding Teenage and Young Adult with cancer
    • Chair the TYA Multidisciplinary Team meeting providing nursing expertise to this forum. Ensure that all patients presented at this forum have a holistic assessment at all key points in their journey and that needs identified are addressed and the impact of these interventions is evaluated.
    • Work with the UHB/ network to co-ordinate the nursing elements of preparation for peer review visits or self-assessment
    • Lead the improvement in nursing care to TYA patients through education, service improvement and service development including transition, aftercare and end of life care and the creation of care pathways and protocols.
    • Participate with service providers in the development and prioritisation of bids to secure funding for cancer services from various agencies, as appropriate, ensuring that proposals are in line with the cancer strategy and needs of Teenage and Young Adult patients.
    • Develop and participate in effective mechanisms which provide clinical supervision to this post as well as other members of the Multi-Disciplinary Team supporting TYAs with Cancer.
    • Contribute to and lead parts of a multimedia communication strategy for patients, patient's network and professionals including the development and provision of patient information.
    • Represent TYA cancer nursing at national forums and committees, including acting as Deputy Lead for the Wales Cancer Network TYA Cancer Site Group
    • Support Teenage Cancer Trust regional fundraising events when required.
    • Establish key working relationships with stakeholders in Principal Treatment Centres, network care providers, primary and secondary care, voluntary and independent sectors, as well as universities orother relevant providers of education to ensure the integration of education, research and practice development.
    • Working with the TYA team, develop processes to ensure that patient/ carer views are at the forefront of all aspects of care, empowering young people to support patient choice.
    • Advise professionals –Nursing, Medical, Allied Health, service managers and commissioners on the benefits of and development of Teenage and Young Adult cancer services.
    • Promote health and well being in Teenage and Young Adults affected by cancer – creating locally supported strategies and tools to encourage this group to take ownership and responsibility for their own well being.
    • Contribute to and further develop the formal and informal TYA educational programme for multi-professional staff that have direct or indirect contact with Teenage and Young Adult with cancer.
    • Contribute to post-registration and post-graduate teaching in TYA as required.
    • Collaborate with colleagues in Teenage and Young Adult cancer research and education to develop meaningful research proposals and projects, which will influence clinical practice.
    • Take a lead role in the audit and evaluation of nursing practice and TYA services, supporting development of national standards, frameworks for Teenage and Young Adult services, implementing quality guidance and Teenage Cancer Trust quality benchmarks.
    • Collate relevant data to provide evidence to influence strategic direction of TYA cancer. Develop and evaluate methodology, which can be usefully interpreted and presented to help shape service direction.
    • Promote cancer care through the publication of papers and presentations at national and international conferences.
    • To utilise up-to-date research and evidence to deliver patient care and to disseminate this knowledge to all members of the TYA multi-disciplinary advisory team.
    • To contribute to the implementation of the strategic Teenage Cancer Trust Nursing strategy.
    • To contribute to the delivery of Teenage Cancer Trust Nursing Metrics and delivery of an Annual Report to the charity.
    • To attend national education programmes/ meetings on behalf of Teenage Cancer Trust.
    • To attend Teenage Cancer Trust events and meetings as requested and agreed with the Charities and the Cardiff and Vale UHB
