    Description

An exciting opportunity has arisen for a Band 5 Occupational Therapist to join the Lower Annandale and Eskdale Home Team. This post would offer an opportunity to work as part of the newly developed home team to support patients at home and aid admission prevention and discharge without delay.
Can you inspire, enable and empower individuals, Carers and colleagues through compassionate conversations to work collaboratively to achieve what matters within improved health and wellbeing outcomes? Can you demonstrate your skills through re-treatment programs, supporting self-management, support mental well-being and maximising independence?
Can you confidently self-coordinate a complex caseload whilst proactively seeking shared best decisions with individuals and multi-disciplinary multi-agency partners? Can you demonstrate effective working relationships across Locality Teams inclusive of GPs, Nursing, Social Work, Social Care, AHPs, Third and Independent sector organisations? Are you aware of Home Teams and how you will be contributing towards working differently in the future to achieve transformational improvements for the health and well-being outcomes for the people of Dumfries and Galloway? It is a very exciting time with transformational change as we move forward with the development of the pathways within rehabilitation and reablement.
If you can answer positively to these questions and feel inspired that this post fits with your own career development plans, we will be equally excited to hear from you.
Please note - A current driving licence is an essential requirement of this post. However, if you have a disability which precludes you from holding a driving licence consideration will be given if alternative transport can be arranged. Due to the ever changing health and social care climate, this post holder may be required in the future to work flexibly within the context of working regular weekends, to provide a 6 or 7 day per week service. There is also a requirement to regularly travel to different workplaces. This will be discussed further at interview.
    Informal enquiries to :-
Rebecca Sorrie – Home Team Lead )

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week for NHS Agenda for Change workers in Scotland from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period, for which you would be paid, until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
