Eliza Assessment Service is seeking to employ an enthusiastic and motivated Comprehensive Assessor to complement the existing multi-disciplinary team.
Practicing within the framework of My Aged Care and within Peninsula Health organisational requirements, the Comprehensive Assessor will provide a comprehensive and independent assessment of the aged client (and some younger adults when appropriate), and with the client develop care plans that promote the restoration of function and/or maximise independence.
The Comprehensive Assessor will also review support plans and facilitate referrals or provide service coordination for older people to engage a range of community or hospital-based services in order to meet their specific needs.
The Mt.Eliza Aged Care Assessment Service is delivered across the boundaries of Frankston, Mornington Peninsula, City of Casey and Cardinia, therefore driving significant distances is a requirement for the position What you need We anticipate that you will have:
