Description
is an exciting model of care that bridges the gap between traditional hospital-based care by providing care in people's home, supported by joint working with system partners and the use of appropriate remote clinical monitoring tools.
You will work closely with the senior team along with other collaborative partners to ensure the service operates efficiently and effectively, ensuring that high standards of patient care are always maintained.
The Band 6 nurse will:
Support the Clinical Operational Lead and B7 with the development, implementation, and delivery of new clinical pathways to support service growth.

Completion of training for additional clinical skills required for service delivery e.g., Experience of undertaking research or audit projects
Evidence of commitment to research-based practice or evidence of project work undertaken
Qualifications and Training
1st Level or other registered healthcare professional
~ Current NMC/ registered healthcare professional Registration
~1st Degree in nursing or other registered healthcare professional
~ Relevant accredited post registration qualification at level 3
~ Current drivers license, be a car owner with a valid insurance certificate covering business use as you will be required to use your own vehicle for daily visits
