    Job summary

    An exciting opportunity has arisen for a band 5 community nurse in the CLDT.

    The team is a forward thinking team that supports people with a learning disability over the 18. We are passionate about the people we support and over the coming years want to embed co-production in the service and ensure people have the equal access to health and social care.

    The Community Nurse is responsible for assessing,planning, coordinating and reviewing a caseload of adults with learningdisabilities. They may have either or both physical and mental health care needs.This will include relationships with Primary and Secondary health care, as wellas private and voluntary care agencies.

    To be responsible for providing clinicalsupport whilst promoting the effective and efficient delivery of high quality outcomefocused service in a person centered approach.

    This is a generic community care roleproviding support and cover across Plymouth city localities.

    There is responsibility forparticipating in reviewing a persons eligibility for Continuing Healthcarefunding and is responsible for making consistent and well-informed professionaldecisions about the determination of CHC funding, ensuring that the nationalframework is applied and they adhere to all local policies and procedures.

    Main duties of the job

    Participating in the undertaking of health needs reviews/assessments to identify needs &plan care accordingly.

    To identify/plan/manage risk of an individuals care needs, relating to physical health/behavioural & or mental health.

    To be responsible for assessing/planning/making recommendations for future care in partnership with patient, carer & multi-professional agencies including case conferences for reviewing the packages of care & care co-coordinating as appropriate.

    To liaise with Primary/Secondary care agencies to ensure better health outcomes for individuals.

    To ensure consideration of Capacity in Best Interest decision making.

    To maintain effective & appropriate communication networks with members of the multi-disciplinary team, the independent sector homes & all stakeholder agencies &organisations to ensure that the individual care needs can be met.

    To contribute to multi-disciplinary team specialist assessments.

    Ability to work as an autonomous practitioner, planning/prioritising/organising workload according to the needs of the service.

    To provide any relevant information/explanation to clients &carers using total communication.

    To provide evidence-based advice/guidance to care staff on the management of health needs to patients & signpost to other services

    To work with all other agencies with regards to the safe guarding process

    All Livewell Southwest staff are expected to able and willing to work across a 7 day service

    Job responsibilities

    1.Tomaintain a high professional standard of conduct at all times in accordancewith NMC/ professional bodies code of professional conduct. Maintain an active status on NMC/professional bodies register.

    2. Tomaintain own re-validation portfolio.

    3.Assumeresponsibility for own professional development and personal knowledge.Undertake and contribute to research where appropriate.

    4.Monitorown performance against agreed objectives through the process of appraisal andpersonal development plans and professional body regulations.

    5.Maintainprofessional expertise by arranging and attending meetings, study days andmandatory training and service training.

    6.Planand participate in the supervision and provision of experience for all nurselearners and other personnel as appropriate.

    7.Participatein regular line management, caseload management and clinical supervision.

    To deliver appropriate training and subsequentsupport to other staff, students and outside agencies when required

    Toact as a role model for other staff and students demonstration high standardsof practice and professional conduct.

    To undertake health promotion/education wheneverpossible to all adults with LD and also to contribute to planned healthpromotion activities

    Experience

    Essential

  • Experience undertaking comprehensive health needs assessments
  • Working in multidisciplinary teams and across health and social care.
  • Desirable

  • Experience of working with people with learning disabilities / mental health issues.

  • Able to demonstrate understanding and competency in assessment, care planning and review.
  • Knowledge of recent developments in Learning disability health care.
  • Understanding of the complexities of physical health care needs in adults with LD.
  • Knowledge and understanding of the political dimensions of health and social care.
  • Knowledge of Personalize care framework
  • Good knowledge and understanding of the Mental Capacity Act, Deprivation of Liberty and reasonable adjustments.
  • Understanding of the safeguarding adults process and relevant issues
  • Desirable

  • Understanding of the work and role of the independent sector and care homes in provision of long-term care
  • Understanding of the continuing care eligibility for fully funded NHS care.
  • Demonstrate an understanding of the Primary Care service delivery.
