Job summary
We are looking for an enthusiastic Band 7 Senior Nurse Practitioner with experience in Community Mental Health, Learning Disability/SEND to join our service. We are an innovative, award-winning, multi-disciplinary community team working with families whose children, including looked after children, have moderate to severe learning disabilities with significant psychological, behavioural and communication difficulties. The team is comprised of Clinical Psychologists, Consultant Psychiatrist, Family Support Practitioners, Specialist Behaviour Support Practitioner, Speech and Language Therapist, Specialist Occupational Therapist and Administrators
We are a friendly and supportive team and you will work creatively to deliver high quality community psychiatric nursing care to a defined caseload. As part of a multi-disciplinary team which provides psychologically informed interventions, you will also broaden or gain direct experience of a wide range of approaches including Systemic, Applied Behavioural, Positive Behaviour Support and the Family Partnership Model.
Main duties of the job
You will be supported within the team by experienced clinicians and will work closely with other specialists within the service, CAMHS and universal children's services.
The post is based in Hailsham, but covers the whole of East Sussex and you will be required to travel between sites. We are a community rather than clinic based service.
The post holder will provide specialist assessments and nursing interventions to patients within a care group; participate in review of clinical policies and guidelines relevant to the area and support and implement changes to practice; liaise with other organisations and professionals to promote communication and good practice; be able to undertake audit activity and supervision to other staff in service area/care group; have an understanding of psychosocial interventions including psychoeducation, wellness recovery action plans and discharge planning. The role will include working with young people to provide assessments & physical health monitoring
