    This is an exciting opportunity for an enthusiasticPharmacist to join our well-established pharmacy team at Severn Health PrimaryCare Network (PCN). Our current team includes 6 Clinical Pharmacists (inc. oneLead Pharmacist), and 4 Pharmacy Technicians (inc. one Lead Technician)

    The role is pivotal to improving quality of care anddelivering operational efficiencies, so it requires motivation and passion forproviding excellent service within general practice.

    Main duties of the job

    The Clinical Pharmacist will optimise medicines, offerhigh-quality, patient-centred, safe, cost-effective medicines, and supportpeople in managing their health, medicines, and long-term conditions. PrimaryCare experience is essential for this role; you must have completed the PCPEPtraining pathway and hold an Independent Prescribing Qualification. You will bewell supported and work under the supervision of the Lead Clinical Pharmacist.

    Severn Health PCN comprises four GP practices based inStroud, Gloucestershire (voted the best place to live in the UK by the SundayTimes in 2021), with a patient population of approximately 39,000. We arelocated near to the M5 motorway allowing an easy commute from nearby towns andcities. The four practices that make up our PCN are: Five Valleys MedicalPractice (Stroud), The Willow Tree Practice (Stonehouse), Price's Mill Surgery(Nailsworth), and Stonehouse Health Clinic.

    The PCN is busy and energetic and has a clear vision for improvingthe health of our patients in an ever-changing environment within oursemi-rural location. The practices within the PCN are very supportive of thepharmacy team. They respect and appreciate the benefits Pharmacists andPharmacy Technicians can bring to patients in primary care, as well as thesupport they can give clinical and non-clinical colleagues.

    Our pharmacy team have come from various employmentbackgrounds, including community pharmacy, hospital pharmacy and CCG work inPrimary Care; all bringing complimentary qualities and transferable skills tothe team. The team is dynamic and enthusiastic and enjoys working together tosupport and share ideas.

    Job responsibilities

    Role Purpose:

    To work within clinical competencies as part of amulti-disciplinary team to provide expertise in clinical medicines management.Provide face-to-face/telephone structured medication reviews, manage long-termconditions, manage medicines on the transfer of care and systems for saferprescribing, manage repeat prescription reauthorisations and acute prescriptionrequests where necessary, support with patients livingin care homes, all whilst addressing both the public health and social careneeds of patients in the GP practices that make up the PCN.

    Main Areas of Responsibility

    As a key part of the Severn Health Primary Care Network(PCN) Multidisciplinary Team, deliver medicines optimisation and offer highquality, safe and cost-effective medicines and patient-centred support, managesome aspects of the quality and outcomes (QOF) framework and PCN targets, withthe support of the Lead PCN pharmacist. The role specifics will include:

    -Undertaking structured medication reviews ontargeted patient groups to ensure patients are taking a safe and effectivemedication regime, using joint decision-making discussions and implementing ownprescribing changes (as an independent prescriber) within scope of practice andordering relevant monitoring.

    -Contributing to NHS priority projects, such asoptimising lipid management in appropriate groups with the use of statins andother newer medications, ensuring the safe use of high-risk drugs, includingDOACs, and identifying and managing patients who may have hypertension.

    -Processing hospital discharge summaries andliaising with patients, GP teams and community pharmacies to ensure anymedicines changes and necessary reviews are carried out, as well as counsellingpatients where necessary and contacting hospital teams if needed.

    -Managing own case load, including runninglong-term condition clinics where responsible for prescribing as an independentprescriber within scope of practice

    -Supporting care homes by reviewing residents,being a point of contact for care home staff and attending multi-disciplinaryteam meetings. Implementing own prescribing changes (as an independentprescriber) within scope of practice and ordering of relevant monitoring tests.Attending and referring patients to multidisciplinary case conferences whererelevant.

    -Advising on and managing the monitoring ofhigh-risk drugs.

    -Supporting the PCN pharmacy technicians whereneeded in handling out-of-stock medication queries by liaising with pharmaciesand providing alternative solutions to GPs if necessary.

    -Supporting the PCN pharmacy technicians in theirwork to carry out regular audits of prescribing safety measures using the GPpractice clinical system.

    -Acting upon MHRA drug updates, alerts andrecalls in conjunction with the PCN pharmacy team, and communicating anyoutcomes to appropriate colleagues as necessary.

    -Answering medication-related queries frompatients and surgery/PCN colleagues

    Data Capture

    Work closely within the Multi-Disciplinary Teams and with GPpractices within the PCN to ensure that information and coding are input intoclinical systems (as outlined in the Network Contract Directed Enhanced Service(DES)), adhering to data protection legislation and data sharing agreements.

    Take appropriate action in accordance with the significanceof the risk and be consistent with protection procedures, applying protectionprocedures, and following lone worker procedures.

    Produce tangible examples of best practice which can behighlighted and shared.

    Demonstrate effective team working inclusive of all relevantprofessionals.

    Report all accidents/incidents, ill health, equipment and/orenvironment failures to line managers.

    Contribute towards audit and data collection as required.

    Adhere to policies and procedures within the PCN, includingconfidentiality, safeguarding, lone working, information governance, and healthand safety.

    Professional development

    Work with your line manager to undertake continual personaland professional development, actively reviewing and developing the role andresponsibilities.

    Work with your line manager to access regular clinical supervision,to enable you to deal effectively with any difficult issues that peoplepresent.

    Review yearly progress and develop clear plans to achieveresults within priorities set by the PCN and you line manager.

    Participate in the delivery of formal education programmes.

    Demonstrate an understanding of current educational policiesrelevant to working areas of practice and keep up to date with relevantclinical practice.

    Key Working Relationships

    Act as a key team member and support effective communicationand stakeholder engagement and management both internally and externally.

    Build professional relationships with key staff in GPpractices within the Primary Care Network (PCN).

    Skills & Knowledge

    Essential

  • Knowledge of IT systems, including the ability to use word processing skills, emails, and the internet to create simple plans and reports
  • Able to obtain and analyse complex technical information
  • Able to identify and resolve risk management issues according to policy/protocol
  • Understand the principles of research governance
  • Able to plan, manage, monitor, and review general medicine optimisation issues in core areas for long-term conditions
  • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
  • Understanding of the wider determinants of health, including social, economic, and environmental factors and their impact on communities
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Able to gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers

  • Experience of working as a qualified Pharmacist in Primary Care, community, prison, or hospital pharmacy
  • Experience and an awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practice
  • Qualifications

  • Masters degree in pharmacy (MPharm) or equivalent
  • Mandatory registration with the General Pharmaceutical Council as a Pharmacist
  • Hold an independent prescribing qualification.
  • Completion of the CPPE PCPEP or equivalent pathway.
  • Demonstrates an understanding of and conforms to relevant standards of practice
  • Follows professional and organisational policies/ procedures relating to performance management
