- Liaise across organisational boundaries with the health care teams in primary care, secondary care and tertiary care to establish systems of care that support the referral and management of patients with heart failure (HF) throughout the patient pathway.
- Demonstrate expertise within the HF field whilst identifying and assessing patients admitted with a diagnosis of HF – with the knowledge and skills to undertake physical assessment, including expediting confirmation of diagnosis with echocardiography as necessary and prescribing appropriate therapy.
- Working in collaboration with community HF nurse specialists, ensure that referrals for the optimum management of their HF are made to relevant members of the MDT.
- Raise the profile of the needs of patients with HF by demonstrating effective communication skills and enthusiasm whilst talking about this innovative role at local and national events.
- Provide support to lead nurse for HF, Cardiac Medicine Matron, senior nurse colleagues/ managers in operational matters as required.
- To participate in nurse-led heart failure clinics in accordance with the developing needs of the service.
- To ensure the development and implementation of protocols and guidance and provide assurance of the provision of evidence-based care.
- To promote service evaluation and development by coordinating and participating in the national HF clinical audit, in addition to seeking and monitoring patient experience of the service. Working for our organisation

An opportunity has arisen for a permanent Senior Specialist Nurse Practitioner within the established heart failure team which has a strong culture of learning together with regular team meetings, MDTs and education sessions.
Based at the Oxford University Hospitals NHS Trust, a small, friendly, and supportive team which is constantly looking at evolving and refining patient pathways and experience for patients admitted with suspected or known heart failure. Covering the John Radcliffe and Horton Hospital sites from Monday to Friday, the post holder may be asked to work flexibly between sites.
A highly experienced, knowledgeable and educated member of the team who is able to work at the level of enhanced practice, applying heart failure specialist knowledge and skills, making decisions to treat a defined caseload of patient's health and care needs.
The role will encompass aspects of education, research and management across the pillars of professional practice, but is firmly grounded in direct clinical care provision or work, with patients', their families and populations. You may be expected to provide leadership, taking an active role in governance and continue the journey of life - long learning.
