    Description

    The Enhanced Pathway Team is a community mental health team operating in South Kirklees and as a Community Psychiatric Nurse (CPN) you will be crucial to the MDT way of working and pivotal in delivering high quality care to service users in the community.

    We have adopted the Flexible Assertive Community Treatment (FACT) model of care for service users who are presenting with increased risk and require a more intensive level of support.

    This feature allows care coordinators to involve the wider MDT in assessing and formulating daily risk management plans and makes the focus on risk a team approach.

    We are looking to recruit an enthusiastic and motivated qualified nurse who can work in a flexible, innovative and creative way.

    The role will primarily be based in Huddersfield and surrounding areas working Monday to Friday 9am - 5pm, however we are keen to support a good work and home life balance so flexible working is available as per the Trusts policy.

    We encourage applications from newly qualified nurses and we offer a robust preceptorship programme to support those who are just entering the workforce.

    This post is a Band 5 developmental post with the opportunity to move into a Band 6 position in line with the appraisal process

    Due to the large geographical are that we cover being a car driver is essential, mileage is available to claim via e-Expenses.

    A key element of the role will be care coordination and MDT working undertaking initial and risk assessment, formulation and reviewing care plans of service user which promote engagement, stability, recovery, and risk management.

    Work with service users with both short-term interventions and severe and enduring mental health needs to promote health and well-being.

    Document care delivery to a high standard in line with professional standards, local and national guidance.

    Recognise situations that may be detrimental to the health and well-being of the service user and initiates action that may be required to manage the situation safely.

    Support student education when allocated to the team and to contribute to the preceptorship and education of newly qualified staff where appropriate.

    Maintain professional registration and demonstrate commitment to statutory code of practice and continued professional development requirements.

    We will offer a robust preceptorship in line with the Trusts policy for newly qualified practitioners and would fully support applications from those who are ending their University studies soon.

    The post holder will be responsible and accountable for the assessment, planning, implementation and evaluation of skilled interventions to address identified needs and risk for allocated clients on a defined caseload.

    To work as a member of an integrated multi-disciplinary team providing comprehensive assessment of clients and their carers, planning, implementing and evaluating skilled interventions to address identified needs.

    ~ To have knowledge of current issues and research relevant to the legislative framework and systems in which mental health care is delivered e.g. To contribute to service development within the clinical governance structure and policies and procedures of the Trust. Assesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action.

    Mental Health Act, Health & Safety, Care Programme Approach, Care Act & Safeguarding.

    Knowledge of working collaboratively with people and their carers to ensure their involvement in the assessment and care planning process.

    Shows awareness of cultural diversity and is able to incorporate this into care planning and the delivery of care.

    Registered Mental Health Nurse / Social Worker / Occupational therapist.

    Can demonstrate skills in assessing and managing people experiencing mental health problems.
    Can demonstrate skills in assessing and managing risk in a variety of settings.
    Ability to support colleagues and supervise students/trainees.

    Experience of working holistically with adults with an emphasis on coexisting mental and physical health needs that impacts on well being.


    Ability to work on own initiative, with reference to policies and procedures.
