    Description

    Are you a Practice Nurse who is passionate about patient care? Are you looking for flexible hours to work around your other commitments? If so, we are recruiting Practice Nurses on a zero hour contract.

    This is an exciting time for PC24 as we are transforming and implementing new models of care across our practices. We are seeking to appoint Practice Nurses to support our 12 practices and our new purpose-built primary care based on Sefton Road.

    About you

    NMC Registered nurse with practice nurse experience. Previous involvement in Primary is essential. This role requires consolidation of specialist knowledge and skills in general practice nursing, demonstrating a depth of knowledge, understanding and competence that supports evidenced informed, complex, autonomous and independent decision-making, and care in general practice and related settings. This role will require personal resilience, clinical leadership, supervision and mentorship of others in the general practice nursing team and providing an effective learning environment for staff and students in the wider team. Key requirements of the role you will be required to undertake:

    • Vaccinations including baby immunisations.
    • Cervical Screening
    • Chronic Disease Management
    • Confident with QOF

    • Shifts available across our 12 practices across Liverpool & Sefton and out of hours and extended access services
    • £18.15ph to £25.67ph depending on previous experience
    • Plus enhanced Rates for unsociable hours

    Hours

    Primary Care Shifts are available between the hours of 08:00 and 18:30 across all 12 of our locations in Sefton and Liverpool. We are a patient centred organisation and therefore require flexibility to cross cover across cluster sites as required:

    Cluster Three

    • Garston
    • Netherley

    Out of Hours face to face shifts vary from hours, 13.00 – 18.00 hours, 09.00 – 17.00, 18.30 – midnight.
