    Job summary

    Undertake a range of nursingassessments and provide appropriate care / treatment in conjunction with GPsaccording to practice policy, protocols etc. Strive to improve standards ofcare and be involved in identifying practice population needs. Chronic Diseasemanagement is an essential part of the role, with a particular focus on themanagement of diabetic patients and the practice approach to this cohort ofpatients to improve health outcomes.

    Assess, plan, evaluate andimplement the care of the patients within the surgery at specified appointmenttimes.

    To be able to work independentlyand within a team.

    To continually develop theservices offered to patients and to continually develop and improve the guidelinesaround high quality clinical practice.

    Child Health ProgrammeImmunisation

    Travel Health Advice andVaccinations (If Qualified)

    Monitoring of Patients withAsthma, COPD (If Respiratory trained), Diabetes, CKD, CHD, Stroke and/ orHypertension in clinics within agreed clinical guidelines.

    Treatment of leg ulcersincluding Doppler

    Cervical Cytology screening

    General contraceptive advice andOral Contraceptive checks, including administering Depo-Provera

    A busy practice in theheart of Crossgates, Leeds 15, looking after around 8,000 patients with a teamof highly skilled GPs, Nursing and Admin/Reception Teams. Our branch surgery is based in Scholes and this is a dispensing practice.

The Diabetic Nurse will play a crucial role in providing specialized care and support to patients with diabetes. The ideal candidate will have a strong background in diabetes management, excellent communication skills, and a passion for delivering high-quality patient care.

  • RGN
  • Team Worker
  • Able to work under pressure
  • Understand and apply the principles of equality of opportunity and access to services
  • Pro-active and able to use own initiative
  • Self motivated

    • Desirable

  • 3 years post registration in hospital or community nursing
  • NMC registered
  • Computer literate
  • Qualifications or continuing professional development in chronic disease management, immunisations, health promotion, cervical cytology, diabetes and wound management
  • Basic knowledge of clinical audit
  • Diploma, degree in Practice Nursing
  • Experienced in the management of diabetic patients and the initiation of treatment in newly diagnosed diabetic patients.
