This is an exciting time to join the Adult Social Care Mental Health Services in Warwickshire as we are transforming how mental health services are delivered.

We recognise the significant impact a specialist social work mental health work force has in delivering social interventions within an integrated community mental health service.

As a result, we are moving towards a specialist social care service provided by Warwickshire County Council and the Coventry and Warwickshire Partnership NHS Trust.


We are looking to recruit 1 Community Connections Practitioner to join the Warwickshire Social Interventions Service which provides a service to people in the community who are experiencing a range of mental health challenges including psychosis, Bipolar Affective Disorder and other mood and anxiety-related disorders.

You will be providing social interventions and working closely with health colleagues in the Community Mental Health Teams. You will receive professional supervision and leadership from a Social Care Lead Practitioner.


For all this post we are looking for a highly motivated, flexible individual who has a proven track record of using a person centred and Strength Based Approach.

You will be providing social interventions that are Strength Based and focused on recovery and a social wellbeing approach.

Your will demonstrate a highly competent level of skill in assessment, planning, intervention and evaluation alongside knowledge of the Mental Health Act, The Care Act and the Mental Capacity Act.


The post holders, as employees of the Council, but seconded to the Trust, will work in close collaboration with our NHS partners in Coventry & Warwickshire Partnership Trust, who provide specialist mental health services.

You will be working within integrated multi-disciplinary teams that include amongst other roles, Community Mental Health Nurses, Approved Mental Health Professionals, Social Workers, Occupational Therapists, Psychologists, Psychiatrists, Support Workers and Clerical Staff.
