    Job summary

    Care Plus Group Primary Care are looking to appoint a Staff Nurse to join the TB Community Service. This is a nurse led service identifying and proactively managing adults and children with a TB diagnosis. The role is embedded within Open Door Primary Care Centre which specialise in offering those patients with highly complex lifestyles, including homelessness, alcohol and drug users and those who present with complex and challenging behaviours which have resulted in them being unable to access mainstream health care provision locally.

    Care Plus Group Primary Care practices provides the first pointof contact for registered patients accessing the health care service. Theservice will work collaboratively with patients to assist them in managing andpreventing illnesses and coordinating access to specialised care. In additionCPG Primary care is part of one of the Local Primary Care Networks (PCN)working in partnership with other practices to offer neighbourhood care acrossa large footprint.

    We are looking for a TB Community Nurse towork as part of an MDT within a nurse led service with responsibility forclinical management of individuals with diagnosed or suspectedTuberculosis. To work under delegationof the Specialist Nurse in the diagnosis and treatment of both adults andchildren including those with dual diagnosis, including HIV and Hepatitis. To be responsible for the provision of nursingadvice and support given to other agencies in relation to TB including thedelivery of education sessions within the local community.

    The successful applicant will provideclinical tasks including venepuncture, injections, DOT therapy in Clinic and incommunity, with an ongoing responsibility to maintain and improve clinicalskills and knowledge as service needs demand.To be responsible for planning and leading BCG clinics includingpreparing delivering and cleaning room ensuring compliance with IPC. To participate in contact tracing for latentor active TB working with Public Health colleagues and other agencies.

    In returnyou will be part of a clinically lead primary care provision, have access tofurther training, receive regular support and be part of a forward thinkingteam

  • Recognised mentor qualification and/or Live on the mentors register OR willingness to work towards
