    Job summary

    Physiotherapist - Intermediate Care Team

    The post holder should have previous experience in working with patients with a wide range of conditions : Neurological, Musculoskeletal / Orthopaedic, and complex elderly conditions.

    You will be expected to work with the team leads to promote an efficient, effective and productive rehab pathway from triage to discharge.

    You will need to be able to work well as part of a wider integrated team and be able to promote innovation and improvement within ICT. You must be well organised and an excellent communicator and be willing to work flexibly. Time management skills and the ability to deal with caseload/staff demands are essential.

    Applicants must have HCPC registration to be considered for this post and have had experience working within the NHS in a clinical role. You will need to be able to travel independently between sites and to patient's homes.

    Main duties of the job

    The post holder will be working with the Intermediate Care Team based at Milford hospital covering the Guildford and Waverley area. The teamcomprising therapists, nurses, Community rehab assistants and Community rehab practitioners, support patients discharged from hospital as part of Discharge to assess pathway(D2A) and also those patients in the community ,managing a health crisis and avoiding unnecessary hospital admission.

    The post holder will have Professional responsibility for themselves and those in which they supervise. They will be expected to contribute to the on-going development of clinical practice, standards, research, as well as policies, protocols, guidelines and service development.

    Job responsibilities

  • To support team leads in the day to day operational management within ICT. This may include planning effectively the case load for oneself and/or the team.
  • To supervise staff and assist in their professional development.
  • To be responsible for the training of Community Rehab Assistants(CRA) and Community Rehab Practitioners(CRP), students, within ICT.
  • To keep up to date with clinical developments, analysing current research and discussing and implementing changes to your clinical
  • practice.
  • To be able to communicate complex and sensitive information to patients, carers and other staff including imparting unwelcome news relating to their rehabilitation potential.
  • To be able to communicate highly complex information sensitively to patients in order to gain cooperation for treatment and valid consent.
  • To use persuading and influencing skills to encourage patients to self-manage their conditions.
  • To appreciate cultural differences and take these into account when agreeing treatment plans with patients.
  • To assess patients understanding of treatment proposals, gain valid consent and have the capacity to work within the legal framework with patients who lack capacity to consent.
  • To use a range of verbal and non-verbal communication tools to communicate effectively with patients in order to progress rehabilitation and treatment programmes. This may include patients who will have difficulties in understanding or communicating, patients who may be hearing or visually impaired, or be unable to accept diagnosis of presenting condition.
  • To use advanced clinical reasoning skills to formulate accurate prognoses and recommend best course of intervention, developing comprehensive treatment and management plans.
  • To evaluate patient progress, reassess and alter treatment programmes as required.
  • To use advanced investigative and analytical skills, manual assessment techniques and diagnostic procedures to assess complex patients and support other more junior staff with their patients as required.
  • To set and maintain a high standard of clinical care using evidence based practice and encourages this culture within the team.
  • To support the ICT team leads in developing strategic and operational management of the service.
  • To support the ICT team leads and senior team in the monitoring of standards and policies of clinical practice within the team.
  • To support designated staff to implement policy and service development changes.
  • To make recommendation to the ICT team leads and senior team around changes in practice supporting implementation of specific changes to improve and update practice or service.
  • To support development of services in line with recent NHS initiatives and guidance.
  • To be proactive in identifying and leading on appropriate projects in developing the service in response to demand.

    Qualifications

    Essential

  • Diploma/ degree leading to inclusion on Health Professions Council Physiotherapy register
  • Evidence of on-going development
  • Desirable

  • Appropriate training at post graduate level or equivalent experiential development
  • Member of the Chartered Society of Physiotherapy
  • Knowledge & Experience

    Essential

  • Post registration experience across a variety of clinical areas within the NHS.
  • Experience of working at highly experienced band 5 or band 6 level with patients in a rehabilitation setting.
  • Experience of working in a multidisciplinary team and contributing to team decisions.
  • Experience and understanding of treatment approaches.
  • Experience of using a range of therapeutic assessments.
  • Experience of use of audit and its use to improve interventions/service.
  • Experience of managing own caseload and prioritising
  • Experience of delegating to junior staff.
  • Experience of teaching junior staff, therapy assistants and other members of the team.
  • Experience of assessing clinical ability of staff and support personal development.
  • Involvement in appraisal system.
  • Demonstrate knowledge of clinical governance agenda
  • Experience of working with complex patients to include Neuro, falls, dementia, orthopaedic and MSK conditions and complex elderly.

  • Previous Community experience.
  • Specific post -registration courses.
  • Experience in appraisal process as appraiser.
  • Clinical supervision of students undertaking post graduate courses.
